全身麻醉复合硬膜外麻醉对肿瘤患者预后影响的Meta分析

目的系统评价全身麻醉复合硬膜外麻醉对肿瘤患者预后的影响。方法计算机检索PubMed、OVID、EBSCO、Cochrane图书馆和CNKI,并手工检索相关杂志以及纳入文献的参考文献,检索时限为1986～2011年。由两名评价者按照纳入与排除标准独立选择试验,提取资料和评估方法学质量后,采用RevMan 5.0软件进行Meta分析。结果最终纳入7个研究,合计2513例患者,7篇文献Jadad评分均在3分以上。Meta分析结果显示:全身麻醉复合硬膜外麻醉与单纯全身麻醉相比,其患者术后转移复发率差异无统计学差异OR=0.71,95%CI(0.44,1.17),P=0.18]。按肿瘤类型、随访时间、术前有无转移和患者年龄4个因素进行敏感性分析,结果显示在高龄组(≥64岁)和短期随访组(≤2年),两种麻醉方式术后复发转移率差异有统计学意义OR=1.46,95%CI(1.00,2.14),P=0.05;OR=1.55,95%CI(1.06,2.26),P=0.02)],但在结直肠癌组和术前无转移组两种麻醉方式的术后复发转移率差异无统计学意义OR=1.00,95%CI(0.62,1.61),P=0.99;OR=1.26,95%CI(0.86,1.86),P=0.23]。结论硬膜外麻醉联合全身麻醉与单纯全麻相比较,不能降低癌症患者术后肿瘤复发转移率。年龄≥64岁和随访时间≤2年的患者应用硬膜外麻醉联合全身麻醉时,其远期复发转移率明显低于单纯全麻;结直肠癌患者和术前无转移患者预后无明显改善。
