丙泊酚复合布托啡诺无痛人工流产术麻醉效果

目的观察丙泊酚复合布托啡诺应用于无痛人工流产术的可行性。方法60例早孕患者行无痛人工流产术,随机分为两组,每组30例。A组静注丙泊酚至睫毛反射消失;B组先静注布托啡诺0．02mg／kg,5min后静注丙泊酚至睫毛反射消失。记录术前、诱导后、扩宫颈、刮宫、术毕的收缩压（SBP）、心率（HR）、血氧饱和度（SpO2）,记录体动反应、丙泊酚用量、苏醒时间、术后1h不良反应及下腹部疼痛程度。结果血压和心率的波动B组少于A组,丙泊酚的用量B组少于A组,体动人数B组少于A组,术后1h疼痛评分B组低于A组,术后头晕发生率B组高于A组,两组苏醒时间和恶心呕吐发生率无统计学上的差异。结论布托啡诺复合丙泊酚用于无痛人流术麻醉效果满意,它可以降低术中体动的发生率,减少丙泊酚用量,不延长苏醒时间。

Propofol combined with butorphanol in painless induced abortion

Objective To observe the feasibility and the security of propofol combined with butorphanol in painless induced abortion. Methods Sixty ASA I voluntary pregnant women undergoing painless induced abortion were enrolled and randomly divided into two groups, with thirty cases in each. Group A： intravenous inject propofol alone until lost eyelash reflex, and group B ： intravenous injected butorphanol 0.02 mg/kg, and 5 minutes later in- travenous injected propofol until lost eyelash reflex. The vital signs as SBP, HR, SpO2 at the preoperation, postinduction, dilation of uterine cervix, uterine curettage and postoperation were recorded. The body movement, propofol dosage, recovery time, adverse reaction and VAS score（one hour after operation） were recorded. Results Compared with those in group A, the fluctuation of the vital signs as SBP and HR was lower, and the propofol dosage, number of body movement and VAS score were less in group B. The incidence of dizziness was lower in group B. The difference of recovery time and incidence of nausea and vomiting has no statistics significance in two groups. Conclusion Butorphanol combined with propofol is effective and secure in painless induced abortion, with reduce propofol dosage and incidence of body movement.
