脉络膜骨瘤的临床及影像学诊断

目的：探讨脉络膜骨瘤的临床及影像学表现，加深对此病的认识。方法：回顾性分析16例18只眼门诊连续就诊病人的眼底、荧光素眼底血管造影（FFA）、B超及CT检查的表现。结果：眼底检查可见肿瘤位于乳头附近或黄斑区，大小不一，呈椭圆形或不规则形，黄白色或桔红色边界清楚，但不整齐，肿瘤边缘有色素脱失。FFA在脉络膜背景荧光显现前病变区出现斑点状荧光，随着视网膜动静脉的充盈逐渐增强，晚期呈弥漫性强荧光。B超检查当声波接触到肿瘤时将出现强反射，形成回声。CT检查眼球后极部眼环上圆形、卵圆形、弧形或半环状光滑锐利的致密影。结论：CT扫描对脉络膜骨瘤的诊断价值最大，但应结合B超、眼底荧光血管造影及眼底表现才能作出正确的诊断和鉴别诊断。

Clinical and Imagenic Diagnosis of Choroidal Osteoma

Purpose:To study the clinical and imagenic characterist ics in choroidal osteoma.Methods:16 cases(18eyes)with choroidal osteoma were examined by indirect ophthalmoscopy,fluorescein angiography,parts of them were examined by B-scan ultrasonography and computed tomography,The dates were retrospectively analysised.Resultes:Ophthalmoscopic examination r evealed a yellowish placoid-like lesion in the peripapillar and macular.Fluorescein angiography showed early phase speckled fluorescein appeared in lesion,late phase allowed assessment of the extension of the osteoma as it is variably hyperfluorescent due to tumor staining combined with a variable degree of overlying retinal pigment epithelial changes.B-scan ultrasonography showed highly reflective lesions.CT scan detected round oval arc and semiround tracks of high refractivity in posterior to the tumor lesion,thick and irregular plate-like,high-signal intensity areas were presented in the region of the tumor.Conclusion:CT scan was most important value in diagnosis of choroidal osteoma.B-scan ultrasonography,fluorescein angiography and funduscope should be combined to make correct diagnosis and differentiation.
