2型糖尿病患者血糖控制对炎性因子的影响

摘    要:目的研究2型糖尿病患者炎性因子与血糖控制质量的关系。方法选择临床确诊的2型糖尿病患者160例.其中男性84例,女性76例;年龄42~73岁.平均年龄63.50岁。根据血糖控制程度分为2组:血糖控制良好组(92例)和血糖控制不良组(68例)。同时选择100例健康人为对照组,其中男性55例,女性45例;年龄45~75岁,平均年龄62.80岁。所有参与者的细胞因子——白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、白细胞介素8(IL-8)浓度应用液体芯片方法检测。结果糖尿病组IL-6、TNF-α、IL-8均显著高于对照组(P〈0.01),且血糖控制不良组IL-6、TNF-α、IL-8亦显著高于血糖控制良好组(P〈0.01)。但IL-1β浓度在对照组与糖尿病组间差异无统计学意义.且与血糖控制程度无关。结论糖尿病患者的细胞因子处于低级别炎症状态.且血糖控制影响炎症水平。

Abstract:Objective To investigate the levels of inflammation in type 2-diabetes patients and their relationship with the quality of blood glucose control. Methods A total of 160 type 2-diabetes patients were enrolled, included 84 males and 76 females, who were aged 42 - 73 years old with a mean age of 63.50. Accorded to the levels of glyeosylated hemoglobin(HbAlc), the patients were divided into 2 groups: well-controlled group(n = 92) and poorly-controlled group(n = 68). One hundred healthy individuals were taken as control group(55 males and 45 females, aged 45 - 75 years old with a mean age of 62.80). The serum concentrations of interleukin-6 (IL-6), tumor necrosis factor-alph a(TNF-α), interleukin-1β (IL-1β) and interleukin-8(IL-8) in all participants were measured simultaneously by multiplexed Luminex assay. Results There was significant difference in serum levels of IL-6, TNF-α and IL-8 between the patient and control group(all P 〈 0.01). Moreover, the levels of IL-6, TNF-α and IL-8 of well-controlled group were higher than that of poorly-controlled group (P 〈 0.01). There was no significant difference in levels of IL-1β between patients and controls. Conclusion It is demonstrated that the cytokine of patients with type 2-diabetes is in a state of low-level inflammation. The change in the serum levels of IL-6, TNF-α and IL-8 is closely associated with the quality of glucose control.
