重症急性胰腺炎的治疗及临床分析

目的：探讨重症急性胰腺炎的合理治疗方案。方法：收集1998年6月至2008年6月经我院收治的120例重症急性胰腺炎病例,并对治疗方案与治疗效果进行回顾性分析。结果：总并发症发生率43.3%（52/120）,总病死率19.2%（23/120）,总治愈率80.8%（97/120）,并发症发生率：保守治疗组为42.4%（28/66）,外科手术治疗组为44.4%（24/54）,P〉0.05;病死率：保守治疗组19.7%（13/66）,外科手术治疗组为18.5%（10/54）,P〉0.05;治愈率：保守治疗组为80.3%（53/66）。外科手术组为81.5%（44/54）,P〉0.05。结论：正确地把握重症急性胰腺炎的治疗时机与指征,合理选择治疗方式,对于决定重症急性胰腺炎的预后至关重要。

The Treatment and Clinical Analysis of Severe Acute Pancreatitis

Objective ： To study the reasonable treatment of severe acute pancreatitis. Method： We collected 120 severe acute pancreatitis in our hospital from June 1998 to June 2008. And retrospectively analyzed the results of treatment program and treatment effect. Result： The total complication rate was 43.3% （52/120） ;the total mortality rate was 19.2% （23/120） ;the total cure rate was 80.8% （97/120）. Complication rate： conservative treatment group was 19.7% （13/66） , surgical treatment group was 18.5% （10/ 54）, P 〉0.05; Mortality rate： conservative treatment group was 30.3% （20/66）, surgical treatment group was 27.8% （ 15/54 ） , P 〉 0.05. Cure rate ： conservative treatment group was 80.3% （ 53/66） , surgical group was 81.5% （44/54） , P 〉 0.05. Conclusion： To grasp the timing and indications correctly for treatment of severe acute pancreatitis and a reasonable choice of treatment is very crucial for determining the prognosis of severe acute pancreatitis.
