高危早期子宫内膜癌术后辅助放疗与化疗的疗效比较

目的：比较高危早期子宫内膜癌术后辅助放疗与化疗的疗效.方法：2002-2006年间,选择我科90例术后高危早期子宫内膜癌患者,分成A组（放疗组）和B组（化疗组）.A组进行全盆野照射放射治疗,B组进行CAP/AP方案化疗,探讨其疗效及不良反应.结果：5年生存率为A组72.9％ （31/43）、B组92.10％（35/38）,差异有统计学意义（P＜0.05）；5年局部复发率为A组6.97％（3/43）、B组15.80％ （6/38）,两者之间无差异（P＞0.05）;5年远处转移率为A组25.58％（11/43）、B组7.89％（3/38）,差异有统计学意义（P＜0.05）；B组近期骨髓抑制较重,A组远期反应明显.结论：高危早期子宫内膜癌患者术后采用辅助化疗优于放疗,能有效控制局部复发及远处转移,提高生存率及生活质量.

The role of postoperative adjuvant radiotherapy or chemotherapy in high risk early endometrial carcinoma

Objective：To evaluate the prognostic value of postoperative radiotherapy or chemotherapy in high-risk early stage endometrial cancer.Methods：2002-2006,90 cases of postoperative high-risk early stage endometrial cancer had been enrolled in this study,and were divided into group A（radiotherapy group） and group B （chemotherapy group）.The patients of A group received whole pelvis radiation field radiation therapy,and group B received CAP/AP regimen chemotherapy.The efficacy and adverse reactions were observed.Results：5-year overall survival rate of A group（72.9％,31/43） was statistically different from that of B group（92.10％,35/38） （P ＜0.05）.There was no statistical difference when comparing the local recurrence rates between A group 6.97％ （3/43） and B group 15.80％ （6/38） （P ＞ 0.05）.The immediate side effects in group was severe bone marrow suppression,in group A patients showed obvious long-term complications.Conclusion：The adjuvant postoperative chemotherapy is superior to radiotherapy in the high-risk early stage endometrial cancer patients.It can effectively control local recurrence and distant metastasis,meanwhile,improve survival and quality of life.
