全面质量管理对Ⅰ类切口手术规范使用抗菌药物的作用

目的 探讨全面质量管理(TQM)对Ⅰ类切口手术预防性规范使用抗菌药物的影响.方法 2012年1月至9月通过实施TQM严格控制Ⅰ类切口手术预防性抗菌药物的使用(干预组),分析Ⅰ类切口手术预防性抗菌药物使用率及抗菌药物使用疗程合理率,并与采用常规管理措施的201 1年1月至9月Ⅰ类切口手术抗菌药物使用情况(对照组)进行比较.结果 干预组Ⅰ类切口手术预防性抗菌药物使用率显著低于对照组(分别为52.1％和89.2％,P＜0.01),抗菌药物使用疗程合理率为82.5％,明显高于对照组(43.6％),差异有统计学意义(P＜0.01).结论 TQM可显著降低Ⅰ类切口手术预防性抗菌药物使用率,提高抗菌药物使用的疗程合理率.

Effects of total quality management on rational use of antibiotics for type 1 incision operations

Objective To explore the effects of total quality management on rational use of preventive antibi-otics for type 1 incision operations. Methods Between January and September 2012, by rigorous implementation of total quality management to curb the irrational use of preventive antibiotics for all type 1 incision operations （interven-tion group）, we analyzed the rates of antibiotic use and the appropriate length of treatment. This allowed subsequent comparison on the use of preventive antibiotics for type 1 incision operations with that using conventional management approaches between January and September 2011 （control group）. Results Compared with control group, the inter-vention group had a significantly reduced use of preventive antibiotics （52.1% vs. 89.2%, P〈0.01） yet markedly in-creased rate of reasonable length of antibiotics （82.5% vs. 43.6%, P〈0.01） for type 1 incision operations. Conclusion Total quality management is associated with reduced use of preventive antibiotics and more reasonable length for ad-ministration of antibiotics for type 1 incision operations.
