人类胚胎冷冻移植的临床应用研究

目的：探讨影响胚胎冷冻结果及冻融胚胎移植后妊娠结局的因素，方法：用常规IVF－ET方法对22例患者进行24个周期的冻融胚胎移植，并对结果进行统计学分析。结果：22例患者24个周期中平均每周期冷冻胚胎5个，解冻后，每周期平均存活4个，继续卵裂3个，平均存活率为83．3％，继续卵裂率为84％，冷冻前，解冻后，移植胚胎3组的胚胎平均评分分别为8．2，8，7．5，组间无统计学差异（P＞0．05），移植后临床妊娠率达22．7％，每个取卵周期的临床妊娠率由28．9％提高至31．7％，患者年龄，胚胎质量，子宫内膜状态与胚胎着床率密切相关，结论：胚胎冷冻技术是对人类辅助生殖技术体系的重要补充和完善，成功的关键在于胚胎质量，适宜的冷冻程序，内分泌环境及子宫内膜发育与胚胎发育的同步化。

The clinical study of human embryo transfer after cryopreservation.

Objective To investigate the factors affecting the results of embryo cryopreservation and the pregnancy results of embryo transfer following thawing.Method 22cases received embryo transfer following thawing in 24cycles under routine IVF-ET procedure.Results There were 4 embryo survived and 3 continously cleaved out of 5 frozen ones following thawing per cycles.The average surviving rate was 83.3% and continue cleaving rate was 84%.The average embryo score before cryopreservation,after thawing and tranfered was 8.2,8.0,7.5,respectively.No statistics differences of embryo score were found among the three groups(P>0 05).The pregnancy rate after transfer arrived at 22.7%,and the clinical pregnancy rate per oocyte retrieval cycle increased from 28.9% to 31.7%.In this study,the embryo implantation rate was closely related with age of patient,embryo quality,and condition of endometrium.Conclusion The technology of embryo cryopreservation is an important supplement for human assisted reproduction technique.The successful pregnancy rate of embryo cryopreservation depends on embryo qualilty,adoptive freeze procedure,endocrine environment and the synchronism between endometrium and embryo development.
