宫腔镜在绝经后子宫不规则出血中的应用

目的:探讨宫腔镜在绝经后子宫不规则出血中的病因及诊治价值。方法:对97例绝经后子宫不规则出血的患者行宫腔镜检查,其中15例行病灶摘除或电切术,切下组织全部送病理。结果:绝经后子宫不规则出血的病因依次为:子宫内膜炎23例(23.71%),子宫内膜息肉19例(19.59%),子宫内膜单纯增生18例(18.56%),萎缩性子宫内膜13例(13.40%),子宫黏膜下肌瘤10例(10.31%),子宫内膜不典型增生8例(8.25%),宫内节育器嵌顿3例(3.09%),子宫内膜癌3例(3.09%)。宫腔镜治疗宫腔良性病变所致子宫出血无一例复发。结论:绝经后子宫不规则出血多为良性病变,宫腔镜诊断绝经后子宫出血准确可靠,部分患者同时行病灶摘除或电切术,减少开腹手术的创伤。

Application of Hysteroscopy in Postmenopausal Anomalistic Uterine Bleeding

Objective To study the value and pathogeny by hysteroscopy for postmenopausal anomalistic uterine bleeding.Methods 97 cases of postmenopausal uterine anomalistic bleeding were inspected by hysteroscopy,among them,15 patients underwent electroresection or fociextirpation.Results Etiology to causing postmenopausal anomalisticuterine bleeding included in turn:23 cases of endometritis,23.71%;19 cases of endometrial polyps,19.59%;18 cases of simple endometrial hyperplasia,18.56%;13 cases of atrophyica endometrium,13.40%;10 cases of1 submucosa myoma,10.31%;8 cases of unrepresentative endometrial hyperplasia,8.25%;3 cases of trauma by intrauterine device,3.09%;3 cases of endometrial carcinoma,3.09%.None patients with benign lesion of uterine cavity relapsed after treatment by hysteroscopy.Conclusion Most of the postmenopausal anomalistic uterine bleeding are benign lesion,diagnoses postmenopausal anomalistic uterine bleeding by hysteroscopy is exact and credible,and meanwhile some patients can avoid trans-abdominal operation,replaced by electroresection or foceextirpation.
