心肺复苏时首剂大剂量肾上腺素的应用研究

摘    要:目的:观察首剂大剂量肾上腺素在心肺复苏中的疗球。方法:51例心跳停搏患者随机分为两组,首剂标准量递增组(A组)25例,首剂大剂量肾上腺素组(B组)26例,各组分别观察自主循环恢复率、首剂心跳恢复有效率、自主循环恢复平均时间、存活率。结果: A组、 B组自主循环恢复率分别为 44%, 76. 9%, B组明显高于 A组(P<0. 05); B组首剂有效率(61.1%)及存活率(65.4%)均明显高于 A组(25%, 32%)(P<0.01和 P<0.05);自主循环恢复平均时间 A组(12. 6±2.5)分钟,显著长于B组(5.4±2.3)分钟(P<0.001)。结论:首剂大剂量肾上腺素比首剂标准量无效后递增肾上腺素能显著提高自主循环恢复率、首剂有效率、存活率,缩短自主循环恢复时间。

Abstract:Objective: It is to observe the curative effects of the high-dose adrenaline for the first on CPR. Methods: The 51 patients after cardias arrest were randomly divided into 2 groups. Group A(n = 25), standard dose and increasing group,received 1 mg adrenaline for the first time, and then used 2mg, 4mg, or 6mg so on at intervals of 4-5 minutes. Group B(n = 26), high-dose for the first, used 0. 1-0. 2mg/kg of adrenaline. Then the authors observed the restoradle rates of the spontaneous citculation, the effective rates of the cardiac movemend for the first dose, and the survival rates in group A and B. Results: The restorable rates of the citculation, the effective rates of the first-dose and the survival rates in group A and B were 44%, 25%, 32% and 76. 9%, 61. 1 %, 65. 4%. The results in group B were higher significant than those in group A (X21= 5. 76,X22 = 5. 51, P<0. 05,X23 = 9. 45, P<0.01). Conclusion: It is concluded that the high-dose adrenaline for the first can yield higher the restorable rates of citculation and the survival rates than the standard-dose and on the increase.
