精索静脉曲张不育症的病理观察及免疫组化研究

目的观察精索静脉曲张(VC)不育症患者睾丸的病理改变,探讨形态学改变及抗精子抗体(AsAb)的表达对睾丸功能的影响。方法在光镜下对睾丸活检进行组织学观察与分析并进行免疫组化研究。结果患者睾丸的病理改变表现为睾丸曲细精管内生精细胞的广泛脱落;曲细精管界膜病变;睾丸间质水肿、间质细胞变性及间质小血管改变;血清中AsAb阳性率明显高于对照组(P<0.01),睾丸组织中IgM、IgG表达明显高于对照组(P<0.05)。结论VC引起的睾丸组织病理改变是导致男性不育症的重要原因,睾丸组织中的免疫反应加重了上述病变,应及早诊断及治疗。

Pathological Observation and Immunohistochemical Study on Varicocele Infertility

Objective: To observe the pathological changes of testis and expression of the antispermatozoal antibody (AsAb) among the varicocele(VC)-induced infertile patients. Methods: 52 cases of patients with varicocele and 10 cases of normal men were ananlyzed by histopathological analysis and Immunohistochemical study. Results: The main pathological change of patients' testis showed that spermatogenic cells widely dropped in the seminiferous tubes. The main pathologic changes of interstitial tissue of testis showed the interstitial edema, interstitial cellular degeneration and change of interstitial blood vessels. Compared with normal men, the positive rates of serum AsAb were significantly higher(P<0.01), and IgG and IgM in testicular tissue among the patients with varicocele were also significantly higher(P<0.05). Conclusion: Pathological change of testis because of varicocele(VC) is an important reason leads to male infertility, and immunological responses quicken up pathological change. It is recommended that it needs diagnosis and treatment early.
