北京地区3050例健康体检者高敏C反应蛋白水平与代谢综合征及其组分的相关性研究

摘    要:目的调查北京地区脑力工作者的代谢综合征(MS)发病率和hs-CRP水平变化情况,探讨hs-CRP在MS患者中的分布特点及其相互关系。方法对2008年3月-2009年3月在我院接受健康体检的3050例(男2345例,女705例)从事管理、科研的脑力工作者进行体重指数(BMI)、腰围(WC)、血压、高敏C反应蛋白(hs-CRP)、空腹血糖(FPG)、糖耐量2h血糖(2hPG)、总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)调查和检测。MS诊断采用国际糖尿病联盟(IDF)标准。结果3050例调查人群中有942例(31.00%)MS患者,男707例(30%),女235例(33.3%)。对应于具有MS组分0、1、2、≥3个的人群,其中男性hs-CRP水平中位数为1.22mg/L、1.47mg/L、1.94mg/L、2.50mg/L,女0.84mg/L、1.03mg/L、1.56mg/L、2.87mg/L,CRP水平随MS组分的增加明显升高(P〈0.001)。Stepwise逐步回归分析显示,男性hs-CRP水平的独立影响因素为BMI、HDL-C、年龄、TC、LDL-C、2hPG(R=01351,R^2=0.123,P〈0.05);女性hs—CRP水平的独立影响因素为BMI、TG、年龄、FBG、TC、HDL—C(R=0.525,R^2=0.276,P〈0.05)。结论北京地区的脑力工作者MS患病率明显高于我国平均水平。随MS异常组分数的增加。hs—CRP水平明显升高,提示hs—CRP是MS发病机制的一部分,是MS发生、发展的一个危险因素,而控制体重、有效干预糖、脂代谢紊乱是降低hs—CRP的重要措施。

Association between high-sensitive C-reactive protein level and metabolic syndrome and components thereof in 3050 brain workers in Beijing

Abstract:Objective The aim of this study was to explore the prevalence of metabolic yndrome(MS) and the level of high-sensitive C-reactive protein (hs-CRP), and study the Association between hs-CRP level and MS and components thereof among brain workers in Beijing. Methods A cross-sectional survey was conducted among 3 050 brain workers (men 2 345, women 705) while physical examination was undergoing in our hospital from March 2008 to March 2009 in Beijing. The BMI, WC, blood pressure, hs-CRP, fasting blood glucose(FPG),2 hours blood glucose(2hPG), total cholesterol(TC),triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) were investigated and determined. The MS was diagnosed according to the International diabetes federation (IDF). Results The prevalence of MS were 31.0%(men 30.0%,women 33.3%)in 3 050 brain workers. Median CRP levels for men with 0,1,2,≥3 components of the MS were 1.22,1.47,1.94 and 2.50mg/L, for women were 0.84, 1.03, 1.56 and 2.87 mg/L, respectively. The hs-CRP level was increased with the components of MS (P〈0.001). Stepwise analysis showed that the BMI,HDL-C,age,TC,LDL-C,2hPG were independent predictors of hs-CRP for men (R=0.351 ,R^2=0.123,P〈0.05), the BMI,TG,age,FBG,TC,HDL-C were independent predictors of hs-CRP for women (R=0.525, R^2=0.276,P〈0.05 ), respectively. Conehltsion Compared with the national average level, the prevalence of MS was significantly high among brain workers in Beijing. The hs-CRP level was increased with the components of MS, which suggested that the high hs-CRP level was a risk factor for the MS occurring and developing. The prevention and control of overweight/obesity and blood glucose and lipids disorder were the most important steps for decreasing the level of hs-CRP.
