摘    要:目的探讨CT引导下臭氧治疗椎间盘源性腰痛的手术技巧及临床疗效。方法对48例椎间盘源性腰痛患者采用局部麻醉下CT引导经皮椎间盘穿刺,向盘内注射浓度为50 mg/L的臭氧20~30 mL进行治疗,并对其临床疗效进行分析。结果 48例患者经13~25个月随访,平均17个月,采用Odom分类系统评定疗效,其中优32例,良9例,可5例,差2例,优良率85.4%,随访中发现随着时间的延长部分患者症状有进一步的缓解,无神经根损伤、切口及椎间隙感染等并发症。结论 CT引导定位臭氧注射治疗操作简单、直观、安全、效果好,且避免医务人员受X线辐射,是治疗椎间盘源性腰痛的有效微创手术。

Abstract:Objective To explore the clinical efficacy and surgical skills of CT guided ozone injection for discogenic low back pain. Methods A total of 48 patients underwent CT guided pertacuneous 03 injection in intervertebral disc under local anaesthesia, and 20-30 mL 03 with the density of 50 mg/L was injected in intervertebral disc. The clincal efficacy was analy- sised. Results All cases were followed up for 13-25 months(average 17 months). According to the Odom evaluation score, the clinical efficacy were rated as excellent in 32 cases, good in 9 cases, general in 5 cases, bad in 2 cases, and the rate of excellent and good outcomes was 85.4%. A part of patients got farther remission as time gone on. No nerve compromise, postoperative infection occurred. Conclusion CT guided ozone injection for discogenic low back pain has the advantages of easy operation, intuition, security, good curative effect, and avoiding X-ray radiation of medical personnel. It was a effective minimally invasive surgery for discogenic low back pain.
