超早期小骨窗手术治疗高血压脑出血48例分析

目的探讨超早期小骨窗手术治疗高血压脑出血的临床效果。方法通过对我院近年来超早期7h内小骨窗血肿清除治疗高血压脑出血48例进行回顾性分析。结果手术48例,术后24h内复查CT,血肿清除均达80%以上,无再出血病例,死亡5例,病死率10.4%。术后3个月按ADL（日常生活能力）分级：Ⅰ级20例、Ⅱ级14例、Ⅲ级5例、Ⅳ级4例、Ⅴ级1例。结论超早期小骨窗手术治疗高血压脑出血,具有创伤小、早期减压效果明显、术后恢复快、能有效降低其病死率和致残率,提高生存质量。

Therapy of Hypertensive Cerebral Hemorrhage by Small Bone Window Craniotomy Operation in Super Early Stage: a Report of 48 Cases

Objective To explore the clinical effects of minimal invasive operation（small bone window penetrations in super early stage） on hypertensive cerebral hemorrhage.Methods A retrospective analysis of 48 cases with hypertensive cerebral hemorrhage undergoing minimal invasive operation in super early stage（within 7 hours） in our hospital these years were carried out.Results Among the 48 cases,the elimination rate of hematoma was over 80%,no hemorrhage cases was found within 24 hours according to the CT recheck,5 cases died（10.4%）.3 months later,20 cases were in Level Ⅰ,4 cases Level Ⅱ,5 cases Level Ⅲ,4 cases Level Ⅳ and 1 case Level Ⅴ according to ADL.Conclusion The treatment of hypertensive cerebral hemorrhage by the small bone window in super early stage has obvious advantages in minimal wound,early significant decompression,rapid postoperative recovery and the effective reduction of the mortality and disability.And can improve the quality of life of patients.
