摘    要:探讨螺旋CT血管造影及三维图像重建(3D-SCTA)在颅内和头颈部病变诊断中的价值。材料和方法:98例头颈部病变患者接受了3D-SCTA检查,其中包括颅内动脉瘤32例,颅内AVM5例,颈动脉粥样梗化35例,脑膜瘤11例,硬膜型海绵状血管瘤2例,颅底神经纤维瘤3例,颈部肿块6例,颈椎椎管内神经纤维瘤4例。结果:全部患者顺利经受螺旋CT血管造影检查,35例颈动脉粥样硬化均显示清楚,且能够检出附壁钙化性

Abstract:Purpose: To evaluate three dimensional spiral CT angiography(3D - SCTA)in diagnosis of head and neck diseases. Materials and Methods: 3D - SCTA were carried out for 98 cases with diseases of head and neck, including intracranal aneurysm(n = 32) , cerebral AVM(n = 5), carotid arteriosclerosis(n = 35), extra - dural cavenous angioma(n = 2), meningioma(n = 11) , skull base neurofibroma(n = 3), tumor of neck soft tissue(n = 6) , and intracanal neurofibroma(n = 4). Results: Thirty of 32 cerebral aneurysms were delineated, including the neck and axis of aneurysm, as well as the supplying artery. 3D - SCTA clearly demonstrated of carotid arteriosclerotic plague. In 16 intracranial tumors the relationship between tumors and surrounding bony structures, such as cervical spine and foramina of skull base, and vessels, such as Willis circle, dural sinus and neck vessels, could be directly demonstrated. The range and conformation of 4 neurofibroma in cervical canal could be detected. 3D imaging reconstruction associated with pseudocolourful demonstration provided imitation of surgical intervention which was of great help for surgeons in preoperative diagnosis and therapeutic decision. Conclusion: 3D - SCTA is an simple, non- invasive , low - cost and reliable imaging technique in the diagnosis and therapeutic decision of intracra- nial, head and neck diseases. It is expected to be the first imaging choice in evaluation of intracranial, head and neck diseases in the near future.
