卡孕栓不同时间给药预防产后出血的临床观察

目的:观察阴道分娩时卡孕栓不同时间置入直肠预防产后出血的效果。方法:将可能有出血因素经阴道分娩产妇300例随机分成3组。A组100例,宫口开全并见胎头的产妇,用卡孕栓1粒(1mg)置入直肠内(深度6cm为宜);B组100例,产妇在胎儿娩出后,用卡孕栓1粒(1mg)置入直肠内(深度6cm为宜);C组100例,产妇在胎儿娩出后肌肉注射催产素10IU。比较3组第三产程时间、产后2h出血量、24h出血总量及产后出血的例数。结果:C组与A、B组比较,第三产程时间、产后2h出血量、24h出血总量均有显著性差异(P<0.05);A组与B组比较,第三产程时间、产后2h出血量、24h出血总量有显著性差异(P<0.05)。A组产后出血2例(2%),B组产后出血5例(5%),C组产后出血9例(9%)。结论:卡孕栓置入直肠用于预防阴道分娩产后出血效果良好,胎儿娩出前用药优于娩出后用药。

Methyl Carboprost-NEGPF Administered at Different Time in the Prevention of Postpartum Hemorrhage

OBJECTIVE:To observe the efficacy of Methyl Carboprost-NEGPF administered per rectum at different time in preventing postpartum hemorrhage in patients with vaginal delivery.METHODS:A total of 300 parturients undergoing vagi-nal delivery with possible postpartum hemorrhage were enrolled:Methyl Carboprost-NEGPF 1 granule(1mg) per rectum (with a suitable inserting depth of 6 cm) were administered in Group A(n=100) when their uterine orifices were fully open and Group B(n=100) after fetal delivery,while Group C(n=100) were injected i.m with oxytocin 10IU after fetal delivery. The three groups were compared in respect of third stage labor duration,postpartum blood loss amount at 2h and 24h and case numbers with postpartum hemorrhage.RESULTS:As compared with Group A and Group B,Group C was significantly different in the duration of third stage of labor and postpartum blood loss amount at 2h and 24h(P<0.05).And there were significant differences between Group A and Group B in the duration of third stage of labor,postpartum blood loss amount at 2h and 24h(P<0.05).2 cases(2%) in Group A,5(5%) in Group B and 9(9%) in Group C had postpartum hemorrhage.CONCLUSI-ON:The efficacy of Methyl Carboprost-NEGPF per rectum was satisfactory for the prevention of postpartum hemorrhage in patients with vaginal delivery,with that administered before fetal delivery superior to that after fetal delivery.
