260例外伤性脊柱脊髓损伤患者致残程度因素分析

目的了解目前本院外伤性脊柱脊髓损伤患者受伤原因和残疾程度现状,探讨相关因素对外伤性脊柱脊髓损伤患者致残程度的影响。方法采用文献检索和专家咨询法收集影响外伤性脊柱脊髓损伤程度的相关因素,然后从受伤原因、是否昏迷、受伤部位保护、运送工具、急救时间、急救医院等级以及首次脊柱脊髓手术时间等方面,设计《外伤性脊柱脊髓损伤致残程度相关影响因素调查表》,培训调查人员,对2010年10月～2011年3月在本院接受康复治疗的260例外伤性脊柱脊髓损伤患者进行访谈调查。结果 260例外伤性脊柱脊髓损伤患者的受伤原因为:交通事故107例(41.2%),坠落伤69例(26.5%),砸伤60例(23.1%),体育伤6例(2.3%),其他18例(6.9%);脊髓损伤程度与损伤水平为:完全性截瘫41.15%,完全性四肢瘫25.38%,不完全性四肢瘫18.08%,不完全性截瘫15.39%。受伤原因在损伤程度和损伤水平各自两个组中有显著性差异(P<0.05);损伤水平在不完全与完全损伤程度两个组有显著性差异(P<0.05);是否昏迷、运送过程中对受伤部位的保护、二次运送在不完全与完全损伤程度两个组中有显著性差异(P<0.05);现场保护情况、运送工具、急救时间、急救医院等级以及首次脊柱脊髓手术时间在不完全与完全损伤程度两个组中均无显著性差异(P>0.05)。结论影响我院收治的外伤性脊柱脊髓损伤患者脊髓损伤水平和损伤程度的因素可能既与受伤原因直接有关,也与伤后不能对受伤脊柱脊髓进行有效保护、早期二次运送救治比率高有关。
