盐酸羟考酮控释片联合加巴喷丁治疗糖尿病性神经痛的临床应用

目的 探讨盐酸羟考酮控释片联合加巴喷丁治疗糖尿病性神经痛的疗效和安全性.方法 将80例糖尿病性神经痛患者按照随机数字表法分成两组,每组40例,A组单纯使用盐酸羟考酮控释片治疗,B组联合使用加巴喷丁,观察比较两组疼痛缓解程度、用药量、生活质量及不良反应情况.结果 两组疼痛均明显缓解,B组治疗后21～28 d的疼痛缓解率明显优于A组95.0%(38/40)比62.5%(25/40)],差异有统计学意义(P＜0.05);B组和A组治疗后28 d的疼痛程度分别为1.2±0.3和2.8±0.5,差异有统计学意义(P＜0.01);B组治疗后28 d,盐酸羟考酮控释片使用剂量低于A组(32.2±4.3)mg比(40.3±5.5)mg],差异有统计学意义(P＜0.05);B组患者的生活质量评分优于A组,差异有统计学意义(P＜0.05或＜0.01);B组治疗后7、14 d头晕发生率高于A组35.0%(14/40)比15.0%(6/40)和30.0%(12/40)比10.0%(4/40)],差异有统计学意义(P＜0.05).结论 盐酸羟考酮控释片联合加巴喷丁治疗中重度糖尿病性神经痛疗效确切,安全可行.

Clinical application of oxycodone hydrochloride controlled-release tablets combined with gabapentin in the treatment of painful diabetic neuropathy

Objective To investigate the efficacy and safety of oxycodone hydrochloride controlled-release tablets combined with gabapentin in the treatment of painful diabetic neuropathy.Methods Eighty patients with painful diabetic neuropathy were divided into group A and group B with 40 cases each by random digits table.Patients of group A received simple oxycodone hydrochloride controlled-release tablets,while patients of group B received oxycodone combined with gabapentin.The intensity of pain, oxycodone dosage, quality of life and adverse effect in two groups was observed and compared.Results The pain was obviously relieved in two groups, and the effective analgesic rate at 21-28 d after treatment in group B was superior to group A95.0% (38/40) vs.62.5% (25/40)], there was significant difference between two groups (P＜0.05).Numerical rating scale NRS score at 28 d after treatment in group B and group A was 1.2 ± 0.3 and 2.8 ± 0.5 respectively,and there was significant difference between two groups (P＜ 0.01 );the oxycodone dosage at 28 d after treatment in group B was lower than that in group A(32.2 ± 4.3 )mg vs.(40.3±5.5 ) mg], and there was significant difference between two groups (P ＜ 0.05 ); the quality of life in group B was superior to group A, and there was significant difference between two groups (P ＜ 0.05 or ＜ 0.01 );the incidence of dizzy at 7, 14 d after treatment in group B was higher than that in group A35.0% (14/40) vs.15.0% (6/40) and 30.0% (12/40) vs.10.0% (4/40)],and there was significant difference between two groups (P ＜ 0.05 ).Conclusion Oxycodon hydrochloride controlled-release tablets combined with gabapentin is effective and safe for the management of painful diabetic neuropathy.
