带蒂大网膜移植治疗食管自发性破裂11例

摘    要:用带蒂大网膜作补片修补食管自发性破裂病人1例。发病至手术相隔13小时至18天,平均大于100小时,均获成功。并从大网膜的生理、解剖及其临床应用诸方面对本术式的可行性进行了讨论。结论:本方法是治疗各期食管自发性破裂的一种疗效可靠、简单易行的术式。

Abstract:Aim:To review the surgical experience of repair of spontaneous esophageal rupture with the pedicled great omentum.Clinical material and method:Eleven patients with spontaneous esophageal rupture were treated surgically.There were 10 males and 1 female with age range of 18 to 57 years.All reptures occurred after vomiting.The most important symptom was epigastric pain.X ray examination showed pneumothorax in 11 patients.The location of rupture was right sided in 7 and left sided in 4.Only two patients were treated less than 24 hours after perforation and 9 patients(82%)were treated longer than 100 hours after rupture.The great omentum was mobilized through median laporatomy and then moved into the thorax via poststernal channel.Repair was through posterolateral thoracotomy and the pedicled great omentum was used to reinforce the suture.Result:There were no hospital mortality and no severe postoperative complications.All 11 patients recovered smoothly.Conclusion:Repair of spontaneous esophageal rupture with the pedicled great omentum is a safe,easy and effective method.
