中晚期宫颈癌同步放化疗近期疗效前瞻性研究

目的：探讨同步放化疗对局部中晚期宫颈癌的临床近期疗效。方法：2004年6月～2008年12月,对局部中晚期宫颈癌随机分成同步放化疗组与单纯放疗组。同步放化疗用PF（DDP＋5-FU）方案并因DDP用法不同分为A组（DDP20mg/m^2·d,连用3d,第1,4周）21例,B组（DDP20mg/m^2·d,每周1次,共6周）23例;5-FU用法相同：0.5/m^2·d,持续6h或以上,连用5d,第1,4周。单纯放疗组45例。结果：治疗结束后3个月观察,同步放化疗组近期疗效优于单纯放疗组,CR、RR比较,差异有统计学意义（P〈0.05）;同步放化疗A组与B组之间差异无统计学意义。主要不良反应为直肠反应和血液学毒性,以Ⅱ～Ⅲ度为主可耐受;无严重不良反应。结论：小剂量PF方案同步放化疗治疗局部中晚期宫颈癌,经济易行,可提高近期疗效。

Prospective trial for concurrent chemotherapy and radiotherapy in medium term and advanced cervical carcinoma

Objective：To investigate the clinical curative effects and side effects of concurrent chemotherapy and radiotherapy for medium term and advanced cervical carcinoma.Methods：From Jun.2004 to Dec.2008,89 patients of medium term and advanced cervical carcinoma were randomly divided into two groups：chemoradiotherapy group,44 patients and only radiotherapy group,45 patients.Each patients of the chemoradiotherapy group was given cisplatin at the dose of 20 mg/m2（A group：d1,per.week vs B group：d1～3,per 4 weeks）,5-Fluorouracil,500 mg/m2,d1～5,repeat every 21 days.And radiotherapy was give at the same time.The patients of radiotherapy group were only received radiotherapy with the same dose and same methods.Results：The treatment effect of the chemoradiotherapy group is superior to radiotherapy group,showing a significant difference（P0.05）.Toxicity of patients was increasing but the patients could tolerate.Conclusion：Concurrent chemotherapy and radiotherapy in medium term and advanced cervical carcinoma can improve the short term curative effect.
