类风湿关节炎早期诊断指标的筛选及鉴定

目的寻找类风湿关节炎的早期诊断指标。方法采用双向电泳、MALDI-TOF/TOF质谱分析等方法从类风湿关节炎滑膜组织中筛选并鉴定特异性自身抗原,进一步建立该抗体的ELISA检测方法,测定30例早期类风湿关节炎患者及30例健康对照者血清中的抗体水平,并分析抗体水平与疾病活动度的相关性。结果早期类风湿关节炎血清中筛选并鉴定出特异性的锰超氧化物歧化酶抗体。ELISA法检测早期RA血清中锰超氧化物歧化酶抗体D(450)值均值(1.72±0.35)明显高于正常人血清(0.95±0.34)(P<0.000 1)。在早期RA患者血清中锰超氧化物歧化酶抗体阳性率为63.3%,且该抗体水平与RF、CCP、CRP呈明显正相关。结论锰超氧化物歧化酶抗体可能作为类风湿关节炎早期诊断与疾病活动性预测的指标之一。

Manganese superoxide dismutase: an early diagnostic marker for rheumatoid arthritis

Objective To investigate an early diagnostic marker for rheumatoid arthritis.Methods Two-dimensional electrophoresis and MALDI-TOF/TOF mass spectrometry were employed to screen and identify the specific autoantigen in the fresh synovial tissues from 2 patients with identified rheumatoid arthritis.ELISA was used to measure the antibodies in the 30 serum samples from patients with early rheumatoid arthritis and the 30 serum samples from healthy donors.The correlation between antibody levels and disease activity was analyzed.Results Manganese superoxide dismutase antibody was specific presence in the serum of early rheumatoid arthritis.The mean D(450) value of manganese superoxide dismutase antibody was 1.72±0.35 in early RA serum,significantly higher than normal serum(0.95±0.34,P<0.000 1).The levels of manganese superoxide dismutase antibody was positively correlated with levels of RF,CCP and CRP in patients with early RA.The positive rate for these serum samples was 63.3% to manganese superoxide dismutase antibody RA.Conclusion Manganese superoxide dismutase antibodies might be used as a biomarker for early diagnosis of rheumatoid arthritis and for prediction of disease activity.
