急性脑血管病患者心脏的应变效应及其机制

摘    要:目的观察急性脑出血(acute cerebral hemorrhage,ACH)和急性脑梗死(acute cerebral infarction,ACI)患者心电图(electrocardiogram,ECG)以及血浆神经肽Y(neuropeptide Y,NPY)含量的变化,探讨急性脑血管病患者心脏的应变机制。方法选择20例ACH患者(ACH组),20例ACI患者(ACI组)作为研究对象,20例健康人作为正常对照组(对照组)。采用放射免疫法测定血浆NPY水平。结果ACH组ECG异常者为16例(80%),ACI组ECG异常者11例(55%),且主要以ST-T改变为主。ACH组血浆NPY水平高于ACI组(P<0.01);两组血浆NPY与对照组比较均显著升高(分别为P<0.05,P<0.01),而ECG异常者血浆NPY水平均分别显著高于ECG正常者血浆NPY水平(分别为P<0.05,P<0.01)。结论NPY参与了脑心综合征的发生发展过程。

Abstract:Objective To observe the changes in electrocardiogram(ECG) and plasma neuropeptide Y(NPY) activity in patients with acute cerebral vascular disease and to explore the mechanism of the cardiac response.Methods Twenty patients with acute cerebral hemorrhage(ACH group),20 patients with acute cerebral infarction(ACI group),and 20 controls(control group) were selected.ECG was performed and plasma NPY was analyzed.NPY activity was determined by radio-immunoassay.Results Sixteen patients in ACH group had abnormal ECG and 11 in ACI group.The plasma NPY activity in ACH group and ACI group was significantly higher than that in control group(P<0.01,P<0.05,respectively),NPY activity in ACH group was significantly higher than that in ACI group(P<0.01).Plasma NPY activity in ACH and ACI patients who had abnormal ECG was significantly higher than that in patients who had normal ECG(P<0.01,P<0.05,respectively).Conclusion Patients with ACH and ACI often accompany with abnormal ECG and plasma NPY activity.The enhanced activity of plasma NPY plays an important role in the mechanism of cerebral-cardiac syndrome.
