原位肝移植10例报告

目的总结解放军150医院肝移植的临床经验。方法回顾性分析2001年4月至2004年5月10例临床肝移植的方法及效果。结果本组肝移植病例围手术期死亡1例，手术病死率为10％。并发症：肺部感染3例，腹腔感染2例，腹腔内出血1例，胆漏2例，应激性溃疡消化道大出血1例，左肝外叶坏死1例。手术存活病例9例，其中1例于术后1年死于肝炎爆发，其余病例术后长期存活，最长1例生存期已达3年。结论确保供肝质量是肝移植成功的前提，良好的血管和胆管重建技术是确保肝移植手术成功的关键，正确的术后处理可有效地减少手术并发症的发生。

CLINICAL ANALYZE IN 10 CASES OF ORTHOTOPIC LIVER TRANSPLANTATION

Objective To summarize the clinical experience of orthotopic liver transplantation in the 150th Hospital of PLA.Methods From April.2001 to May.2004,there were 10 patients underwent liver transplantation.The data from those patients were restrospectively analyzed.Result 1 cases died during perioperation,the mortality was 10%.Complications included:pulmonary infection(3 cases),intraperitoneal infection(1 cases),intraperitoneal bleeding(1 cases),bile leakage(2 cases),hemorrhage of upper digestive tract(1 cases),and necrosis of left lateral lobe of liver(1 caes).The longest survival time was 3 years.Conclusion The successful liver transplantation relies on good quality of liver graft.Excellent vascular and bile duct reconstruction is the keystone of liver transplantation.Proper postoperative management can effectively reduced the complications of operation.
