不同b值扩散加权成像对肝脏良恶性占位的鉴别诊断价值分析

目的:分析不同b值扩散加权成像对肝脏良恶性占位的鉴别诊断价值。方法:选取2017年10月~2019年2月在本院治疗的50例肝脏恶性占位患者(A组)和52例肝脏良性占位患者(B组)。对其均行MRI平扫及b值DWI检查,观察并比较两组患者DWI信号特征以及不同b值下ADC值。结果:一般情况下,肝脏良恶性占位患者DWI信号特征以高信号为主;当b值取2000s/mm^2时,A、B两组患者的DWI信号特征存在着明显差异(P<0.05);当b值分别取1000、2000s/mm^2时,A组患者的ADC值均低于B组,差异明显(P<0.05);随着b值的升高,ADC值对肝脏恶性占位诊断的ROC曲线的曲线下面积逐渐扩大,诊断价值越高。结论:DWI检查对肝脏良恶性占位的鉴别诊断价值较高,当b=2000s/mm^2时,DWI检查对肝脏良恶性占位的鉴别诊断价值优于b=600、1000s/mm^2时的诊断价值。

Differential Diagnostic Value of Different B-value Diffusion Weighted Imaging for Benign and Malignant Liver Mass

Objective:To analyze the value of diffusion-weighted imaging with different b-values in the differential diagnosis of benign and malignant liver occupation.Methods:50 patients with malignant liver occupation(group A)and 52 patients with benign liver occupation(group B)who were treated in our hospital from October 2017 to February 2019 were selected.MRI plain scan and b-value DWI examination were performed on all patients,and the DWI signal characteristics and ADC values under different b-value were observed and compared between the two groups.Results:In general,the DWI signal characteristics of patients with benign and malignant liver occupation were mainly high signal.When b value was 2000s/mm^2,there were significant differences in DWI signal characteristics between A and b groups(P<0.05).When b values were 1000 and 2000s/mm^2,ADC values in group A were lower than those in group b,with significant difference(P<0.05).With the increase of b value,the area under the ROC curve of liver malignant occupation diagnosed by ADC value gradually expanded,and the diagnostic value increased.Conclusion:The differential diagnosis value of DWI examination for benign and malignant liver occupation is higher.When b=2000s/mm^2,the differential diagnosis value of DWI examination for benign and malignant liver occupation is better than that of b=600 and 1000s/mm^2.
