抑郁症患者有无自杀行为的临床对照研究

目的　了解抑郁症患者的自杀行为。方法　对 1 992～ 2 0 0 3年在洛阳市精神卫生中心及我院住院病例 ,由 2名主治医师对住院病历进行评估。入组条件 :1符合 CCMD-2 -R抑郁发作诊断标准 ;2 HAMD>1 8分 ;3排除脑器质性疾病及躯体精神障碍 ;4无药物滥用及酒精依赖史。符合抑郁诊断病历 2 42例 ,其中男 92例 ,女 1 5 0例 ,有自杀行为者 (简称“自杀组”) 1 0 5例 (占 43 .4% ) ;无自杀组 (简称“无自杀组”) 1 3 7例。结果　自杀组中 ,生活事件作为诱发自杀因素中最为多见 ,两组有显著性差异 (χ2 =1 9.3 42 ,P<0 .0 0 5 ) ;就临床症状而言 ,绝望、自责自罪、焦虑、妄想及早醒等以自杀组显著较多 ( P<0 .0 0 5～ 0 .0 5 ) ,自杀组女性多于男性 ,两组比较有显著性差异 ( P<0 .0 5 ) ,而两组疗效、住院天数及病程无显著差异 ( P>0 .0 5 )。结论　对抑郁症有自杀行为者应进行积极防范和治疗 ,特别对病前有不良生活事件及睡眠障碍 (早醒 )、焦虑、绝望、自责自罪、妄想等症状者 ,应引起社会、家庭及精神科医务工作者的重视

The Control Study of Suicidal Behavior or no Suicidal Behavior with Depression Patients

Objective To explore suicidal behavior with depressive patients.Methods 242 patients with depression were divided into two groups.Male are 92 and female 150.Suicidal behavior group(n=105)and no suicidal behavior group(n=173).Results Life events are more than other factor in suicidal behavior group.Suicidal behavior group's female is more than male.There are significant differences in two groups(P<0.05).Conclusion The clinical mental doctor should pay attention to depressive of the suicidal behavior.It's very important that observe the patients carefully and afford effetive therapy.
