奎硫平联合碳酸锂治疗双相抑郁患者的疗效

目的:探讨奎硫平联合碳酸锂治疗双相抑郁患者的疗效和不良反应. 方法:将符合中国精神障碍分类和诊断标准第3版双相情感障碍抑郁发作诊断标准的64例患者随机分为奎硫平合并碳酸锂组(研究组)和氟西汀合并碳酸锂组(对照组).疗程8周.临床评定采用汉密尔顿抑郁量表(HAMD)及汉密尔顿焦虑量表(HAMA),用治疗中出现的症状量表(TESS)评定不良反应. 结果:治疗8周,研究组和对照组有效率分别为84.4％、78.1％,痊愈率分别为53.1％、46.9％,两组疗效差异无统计学意义(分别为t =0.10,P＞0.05；t=0.25,P＞0.05)；研究组患者抑郁症状于第2周末明显改善,而焦虑症状于第1周末即显著好转；治疗8周末两组TESS评分差异无统计学意义(t=0.46,P＞0.05).结论:奎硫平联合碳酸锂治疗双相抑郁疗效明确,使用较为安全,而且起效较快.

Analyses of quetiapine combined with lithium carbonate in the treatment of bipolar depression

Objective:To explore the effect and adverse reaction of quetiapine combined with lithium carbonate on treatment of depression.Method:64 patients meeting the criteria of depression in CCMD-3 were randomly divided into study group (treated by quetiapine combined with lithium carbonate) and controlled group (treated by fluoxetine combined with lithium carbonate).The course of treatment was 8 weeks.Therapeutic efficacy was measured by HAMD and HAMA and adverse reaction was rated by TESS.Results:The effective rate of the study group and controlled group was 88.4%,78.1%,respectively.The cure rate of the study group and controlled group was 53.1%,46.9%,respectively.No significant differences between the effective rates of the two groups was found (t=0.10,P>0.05;t=0.25,P>0.05);In study group,depression symptoms improvement occurred significantly at the end of the 2nd week,and anxiety symptoms improvement at the end of the 1st week.The score of TESS was similar between the two groups after 8 weeks treatment (t=0.46,P>0.05).Conclusion:Quetiapine combined with lithium carbonate was effective and safe in the treatment of depression with early response.
