乙酰半胱氨酸联合纤支镜治疗儿童机械通气重症肺炎的疗效

目的:分析乙酰半胱氨酸联合纤支镜治疗儿童机械通气重症肺炎的疗效。方法:研究阶段为2015年5月30日—2018年5月30日,共纳入研究对象98例,均为机械通气重症肺炎患儿,根据入院顺序的先后分为两组。对照组(n=49)采用纤支镜肺泡灌洗联合氨溴索治疗,观察组(n=49)采用纤支镜肺泡灌洗联合乙酰半胱氨酸治疗,比较两组临床效果。结果:观察组机械通气时间(6.49±2.99)d、住院时间(11.24±2.16)d均短于对照组机械通气时间(8.61±4.23)d、住院时间(13.82±2.35)d,差异显著具有统计学意义(P<0.05),两组28 d病死率差异不显著,无统计学意义(P>0.05)。观察组感染控制率95.92%(47/49)、治疗总有效率91.84%(45/49)均明显高于对照组感染控制率81.63%(40/49)、治疗总有效率83.67%(41/49),差异显著具有统计学意义(P<0.05)。结论:在治疗重症肺炎患儿中,应用乙酰半胱氨酸联合纤支镜的治疗效果显著,因此,值得积极推广与应用。

Effect of acetylcysteine combined with fiberoptic bronchoscopy on severe pneumonia in children undergoing mechanical ventilation

Objective:To analyze the effect of acetyl cysteine combined with fiberoptic bronchoscopy in the treatment of severe pneumonia in children with mechanical ventilation.Methods:From May 30,2015 to May 30,2018,98 cases were included in the study,all of them were divided into two groups according to the order of admission,and the control group(n=49)used fibrinocyst.The observation group(n=49)was treated with bronchoalveolar lavage and acetyl cysteine in the observation group(n=49),and the clinical effects were compared.Results:The time of mechanical ventilation was(6.49±2.99)d,and the time of hospitalization(11.24±2.16)d.The duration of mechanical ventilation was(8.61±4.23)days,the hospitalization time was(13.82±2.35)days,the difference was statistically significant(P<0.05).There was no significant difference in 28 days mortality between the two groups(P>0.05).The infection control rate of observation group was 95.92%(47/49),the total effective rate of treatment was 91.84%(45/49)significantly higher than that of control group 81.63%(40/49).The total effective rate of treatment was 83.67%(41/49).The difference was statistically significant.(P<0.05).Conclusion:The application of acetylcysteine combined with fiberoptic bronchoscopy in children with severe pneumonia can shorten the time of mechanical ventilation and has good clinical application value.
