盐酸氯普鲁卡因用于连续臂丛神经阻滞的临床观察

目的 :利用国产盐酸氯普鲁卡因进行连续臂丛神经阻滞 ,观察其麻醉效能。方法 :选择 6 0例上肢手术病人 ,随机分为实验组和对照组 ,实验组用 2 %盐酸氯普鲁卡因 ,对照组用 1 5 %利多卡因 ,均采用肌间沟臂丛神经阻滞 ,用 2 2G静脉留置针穿刺产生异感后将留置针向前推进 2mm ,拨出针芯 ,用丝线将留置针尾部与皮肤固定 ,连接带延长管的输液三通 ,先分别注射 3mL试验量 (不含肾上腺素 )。 6min后注射 9mL(含 1:2 0万U肾上腺素 )。观察麻醉起效时间、痛觉消失时间、运动阻滞时间、痛觉恢复时间、运动恢复时间以及呼吸、循环的变化等。结果 :麻醉起效时间、痛觉消失时间、运动阻滞时间、痛觉恢复时间在实验组均短于对照组 ;运动恢复时间实验组快于对照组 (P <0 0 5 ) ;两组呼吸、循环变化无明显差异 ,均未见明显不良反应。结论 :国产盐酸氯普鲁卡因起效快 ,神经阻滞完善 ,作用时间短 ,无明显不良反应 ,配合连续臂丛神经阻滞能够满足各种上肢手术 ,不受时间长短限制 ,并能随时检测手术效果

Efficacy of Domestic Chlorprocaine Hydrochloride in Continuous Brachial Plexus Block

Objective:To evaluate the efficacy of domestic chlorprocaine hydrochloride in continuous brachial plexus block. Methods: 60 cases scheduled for arm surgery were divided randomly into two groups. 2% chlorprocaine was given to test group and 1.5% lidocaine was given to control group. Respectively, with intrascalene approach. The intravenous needle 2mm was pushed after the patients sensed abnormal, then fixed it. First every patient 3mL(did not contain adrenaline)was given, 6 minutes later, 9mL(contain 1:200000U adrenaline) was given.Then recorded the onset time , pain free time, motor free time, pain recovery time, motor recovery time and the changes of circulation and respiration. Results:The onset time, pain free time, motor free time and pain recovery time of continuous branchial plexus block in test group were shorter than those in control group.The motor recovery time was shorter in test group than that in control group(P<0.05). The changes of respiratory and circulatory parameters were similar in both groups, and side-effects were not observed in both groups. Conclusion: Domestic chlorprocaine hydrochloride is quick in onset, its efficacy is well, recovery time shorter and no adverse reaction in continuous brachial plexus block. It can be used safely for continuous brachial plexus block in patients scheduled for arm surgery and its efficacy can be observed at any time when operation.
