核酸体外扩增技术诊断骨结核病的临床研究及应用价值

目的 探讨核酸体外扩增(polymerase chain reaction,PCR)技术在骨结核病诊断与鉴别诊断中的应用价值。方法 对60例骨结核病标本与30例非骨结核病标本分别进行核酸体外扩增技术,抗酸染色镜检及结核杆菌分离培养法检测结核杆菌,分析影响核酸体外扩增技术结果的相关因素及处理方法。结果 骨结核标本结核杆菌检出阳性率:PCR法83％,抗酸染色镜检法3％,培养法7％。非骨结核标本结核杆菌检出阳性率:PCR法10％,其他方法均未检出,经统计学处理P<0.005,PCR法与镜检法及培养法相比较差异具有显著性。结论 PCR技术是一种快速、敏感、简便、特异的检测骨结核标本结核杆菌的方法,明显优于镜检法及培养法,在骨结核的诊断与鉴别诊断中有重要价值。

Clinical Study and Value for Polymerase Chain Reaction Technique in Diagnosis of Bone Tuberculosis

Aim:To evaluate the clinical value for polymerase chain reaction (PCR) technique in diagnosis and differentiating diagnosis of bone tuberculosis. Method: PCR, standard light - microscopy and standard culture technique were used to detect mycobacterium tuberculosis in samples obtainecd from 60 patients with bone tuberculosis and 30 patients without bone tuberculosis. In the meantime, some factors affecting PCR result were analysed and methods to deal with them were discussed. Results: In the group of 60 patients with bone tuberculosis, the positive rate was 83 % in PCR technique, 3 % in standard light - microscope technique and 7 % in the standard culture technique (P<0.005). In the group of 30 patients without bone tuberculosis,2 cases showed positive in PCR technique,none in the other methods. Conclusion:PCR technique is a rapid,specific,sensitive and simple method for detection of mycobacterium tuberculosis in sample of bone tuberculosis, and it is of great value in the diagnosis of bone tuberculosis and differentating bone tuberculosis from other bone diseases.
