| 摘 要： | 目的：观察在常规治疗的基础上，加用肾上腺素（小剂量静脉持续泵入）治疗顽固性心力衰竭的疗效。方法：将44例顽固性心力衰竭患者随机分为肾上腺素组和多巴胺组，每组各22例，两组在常规治疗基础上，分别使用微量泵静脉输注小剂量肾上腺素或多巴胺，治疗36—72h。治疗期间观察心率（HR）、血压（BP）、脉搏血氧饱和度（SpO2）、尿量及心功能情况，治疗前后行超声检测左室功能。结果：肾上腺素组显效率和总有效率高于多巴胺组。但差异无统计学意义（P〉0．05），用药后肾上腺素组每分输出量（CO）、心脏指数（CI）和左室射血分数（LVEF）高于多巴胺组，差异有高度统计学意义（P〈0．01）。结论：短期静脉输注小剂量肾上腺素，在慢性心力衰竭加重时有助于改善患者心脏功能，能起到帮助患者渡过危险期的作用，疗效优于多巴胺。
