腹腔液CEA和组织CD44v6表达与胃癌转移关系的研究

目的　探讨转移相关基因CD4 4v6 及腹腔液CEA与胃癌生物学行为及转移的关系。方法　采用免疫组化方法检测了 49例进展期胃癌标本中CD4 4v6 表达以及患者腹腔液中CEA水平 ,分析其与胃癌转移的关系。结果　胃癌标本中的CD4 4v6 表达与胃癌分化程度显著相关 (P <0 0 5 ) ,与肿瘤侵及浆膜及胃周淋巴结转移密切相关 (P <0 0 1) ;腹腔液CEA阳性率与肿瘤大小、肿瘤分型、肿瘤分化程度、侵及浆膜、淋巴结转移均密切相关 (P <0 0 1)。结论　CD4 4v6 在胃癌的癌细胞分化发展过程和淋巴结转移中起着重要的作用 ,腹腔液CEA升高以及CD4 4高表达对早期预测胃癌细胞的转移、判断预后及指导术后综合治疗具有重要的临床意义。

EXPRESSION OF CD44v6 IN CANCER TISSUES AND DETECTION OF CEA IN PERITONEAL FLUID OF GASTRIC CANCER PATIENTS PREDICTING THE METASTASIS

Objective To study the expression of CD 44v6 in gastr ic cancer tissues and detection of CEA in peritoneal fluid of gastric cancer patie nts and their significance for predicting the metastasis. Methods CEA was detected by radioimmunoassay. CD 44v6 was done by immunohistoc hemical SP technic. These cases were collected during operation from 49 patients with gastric cancinoma. Results It was found that CD 44v6 expression was present in 80% of low-differentiational cancers while it was only 53% in high-differentiational cancers (P<0.05). The CD 44v6 exp ression in cases with lymph node metastasis and the serosal involvement was sign ificantly higher than that withoutit (P<0.01). Lymph node metastasis, sero sal involvement, tumor differentiation and size of tumor were related to the pos itive rate of CEA (P<0.01). Conclusions These data sugg est that the expression of CD 44v6 in gastric cancer tissues and detection of CEA levels in peritoneal fluid are of significance in predicting the potentia l cancer cell metastasis,prognosis and dectermining postoperative treatment.
