摘    要:目的探讨经皮扩张气管切开术(PDT)在急性吉兰-巴雷综合征(Guillain-Barre syndrome,GBS)致呼吸肌麻痹中的应用价值。方法1996年1月—2006年1月,我科共收治急性GBS综合征致呼吸肌麻痹患者16例,其中行PDT10例,并与常规机械通气组6例进行比较。结果两组在机械通气时间、住院时间、住院期间呼吸机相关性肺炎的发生率、存活率等方面有显著差异(P<0.05)。结论PDT用于急性GBS综合征致呼吸肌麻痹患者具有操作简单、通气时间、住院时间短、住院期间呼吸机相关性肺炎的发生率低等优点,应作为该病开放呼吸道的首选方法。

Abstract:Objective To observe the applicative value of pass-skin dilatation tracheotomy (PDT) in the inpatients of Guillain-Barre syndrome (GBS) with respiratory muscle paralysis. Methods A total of 16 inpatients of GBS with respiratory muscle paralysis, who were admitted to our department from Jan. 1996 to Jan. 2006, were divided into two groups, i.e. 10 inpatients (Group A) underwent PDT and six (Group B) underwent machine ventilation. Results The two groups differed evidently in ventilating time, hospitalization duration, occurrence rate of pneumonia in the hospital, livability, etc (P<0.05). Conclusion The application of PDT to the inpatients of GBS with respiratory muscle paralysis has advantage such as simple operation, short ventilating time and short hospitalization duration, low occurrence rate of ventilator related pneumonia, etc. It should be considered as the first-choice method to opening the air-way for the disease.
