股骨近端锁定加压钢板与Gamma3TM钉内固定治疗股骨粗隆间骨折的比较

目的:比较股骨近端锁定加压钢板与Gamma3TM钉内固定治疗股骨粗隆间骨折的疗效。方法:2005年1月至2009年6月间在我院治疗的随访资料完整的股骨粗隆间骨折72例,采用股骨近端锁定加压钢板(LCP组)治疗35例,行Gamma3TM钉治疗(Gamma3TM组)37例。记录和比较两组患者的手术时间、术中出血量、骨折愈合时间及末次髋关节功能评分等相关数据。结果:LCP组平均手术时间97.6 min、术中平均出血量为416.6 ml,Gamma3TM组平均手术时间46.8 min、术中平均出血量为141.2 ml。两种内固定手术方式在手术时间、术中出血量方面差异均有统计学意义(均P<0.05),在骨折愈合时间、髋关节功能评分方面差异均无统计学意义(P>0.05),不同骨折类型与术后髋关节功能评分相关(P<0.05)。结论:两种内固定方式都是治疗股骨粗隆间骨折的有效方法,但Gamma3TM钉创伤相对较小,有一定的优势。

A comparative study of proximal femoral locking with compression plate and Gamma3TM nail in the treatment of femoral intertrochanteric fractures

Objective:To compare the therapeutic effect on femoral intertrochanteric fracture with proximal femoral locking compression plate(LCP) and Gamma3TM nail.Methods:A retrospective study of 72 cases with femoral intertrochanteric fracture were carried out,from January 2005 to September 2009,35 cases were treated with LCP internal fixation,and 37cases with Gamma3TM nail.The date of operative time,operative blood loss,union time and hip function scores(Harris scores standard) were recorded.Results:The mean operative time of LCP group was 97.6 minutes.The mean operative blood loss of LCP group was 416.6 ml.The mean operative time of Gamma3TM group was 46.8 minutes.The mean operative blood loss was 141.2 ml in Gamma3TM group.There were significant differences in operative time,operative blood loss in LCP and Gamma3TM nail internal fixation treatment for femoral intertrochanteric fracture(P<0.05).There were no significant differences in union time and Harris score(P>0.05).But Harris score was significantly relative to the types of fracture(P<0.05).Conclusion:Two kinds of internal fixation are effective way to treat femoral intertrochanteric fracture.But Gamma3TM nail has more advantages in reducing trauma.
