15例小儿危重型甲型H1N1流感临床分析

目的:探讨儿童危重型甲型H1N1流感临床特点。方法:对我院15例危重型甲型H1N1流感患儿临床资料进行回顾性分析。结果:年龄最小7d,最大11岁,<5岁者占93.33%;13例患儿均无明确甲流病人接触史;3例有基础疾病;主要表现为呼吸困难、发热、咳嗽喘息,肺部呼吸音均有异常,胸片主要为肺炎或ARDS改变;治疗以奥司他韦抗病毒、呼吸支持等为主。死亡1例,放弃治疗2例,病情好转或痊愈出院12例。结论:儿童各年龄段均可发病,多无明确甲流病人接触史;<5岁、有基础疾病者易进展为危重型;危重病人呼吸困难显著;应用奥司他韦抗病毒等综合治疗有效,治疗过晚、免疫功能低下者预后不良。

Analysis of clinical characteristic of critically influenza A (H1N1) in 15 pediatric patients

Objective:To explore clinical characteristics of critically influenza A(H1N1) in children.Methods:Clinical characteristics of critically ill children with H1N1 in Zhengzhou Children’s Hospital were analyzed using a piloted case report form.Results:The youngest age is 7 days old and the biggest is 11 years old.There are 93.33% patients’ ages below 5 years old and 13 cases with no contact with A(H1N1) patients.There are 3 cases with base illness.Their clinical characteristic are include breath difficulty,fever,cough and asthma.All of them have abnormal breath sounds.X-ray of them presented pneumonia and acute respiratory distress.All patients were given oseltamivir,breath support therapy and so on.One patient died,and two patients abandoned therapy,and 12 patients were discharged with the disease cured.Conclusion:All ages in children can get A(H1N1) and with no contact with A(H1N1) patients.The risk factors of severe illness include the years below 5 years old and base illness.Critically A(H1N1) in children presented difficult breath.It’s effective that we gave oseltamivir and other support therapy.The prognosis is not good for such patients that received therapy lately and had immunologic hypofunction status.
