髋臼骨折的手术及康复治疗

目的：报告移位髋臼骨折手术及康复治疗的临床经验。方法：自１９９４ 年３ 月～１９９８ 年５ 月间，２２ 例移位髋臼骨折采用Ｋｏｃｈｅｒ－ Ｌａｎｇｅｎｂｅｃｋ 入路、髂肌延长入路及髂腹股沟入路三种入路手术整复固定，术后第３ ｄ开始应用下肢ＣＰＭ机治疗及主动下肢肌肉功能训练。结果：２２ 例全部获得随访，平均随访１６ 个月。以美国矫形外科医生学会髋关节功能评定标准评定，优８ 例，良１０ 例，可２ 例，差２ 例，优良率８１．８２％ 。结论：移位髋臼骨折手术整复固定，配合早期有规律、有限制康复训练可收到良好的治疗效果。

Operation and rehabilitaion treatment of acetabular fracture

To report the clinical experience of the operation and rehabilitation treatment of displaced fracture of the acetabulum .Methods:From march 1994 to may 1998,22 cases of displaced fracture of the acetabulurn were operated on with three surgical approaches,Kocher-Landgenbeck,ilioinguinal ,or extended iliofemoral.From the third postoperative day,all patients were treated with CPM machine for two weeks and were allowed to exercise the muscles of the affected extremity.Result:22 Cases were followed up for 16 months on an average.According to the criteria of hip joint function evaluation set up by the America Academy of Orthopaedic Surgeons,excellent were found in 8 cases,good in 10 cases, fair in 2 cases and poor in 2 cases.The excellent and good rate was 81.82%.Conclusion:Open reduction and internal fixation and early, regular and limited treatment of rehabilitation of displaced fracture of the acetabulum can lead to better result.
