小切口超声乳化及大直径折叠式人工晶体植入治疗高度近视眼白内障临床观察

目的评价小切口超声乳化及大直径折叠式人工晶体植入对于治疗高度近视性白内障的安全性和疗效。方法表麻醉下对40例68只眼高度近视白内障患者施行透明角膜3.2mm切口超声乳化吸除,植入大直径后房型折叠式人工晶体。察术中的安全性及术后疗效。结果术中无一例发生后囊膜破裂、前房出血以及爆发性脉络膜上腔出血。术后1天裸眼或正视力≥0.5者,占54.41%;术后1周为70.59%;1个月为82.35%;视力≥1.0者,术后1天为8.82%、术后1周为25%、1个为41.18%。结论小切口超声乳化及大直径折叠式人工晶体植入治疗高度近视白内障,具有视力恢复快、无严重并发症等点,但是对手术技巧要求较高。术后视力提高与高度近视眼底病变有关。

Clinical Observation of cataract operations with high myopia by small incision phacoemulsification combined with big-diameter and foldable intraocular lens implantation

Objective To evaluate the safety and therapeutic effects of cataract operations with high myopia by small incision phacoemulsification combined with big-diameter and foldable intraocular lens implantation. 68 eyes of 40 cataract patients with high myopia were treated by phacoemulsification by 3.3 mm clear cornea incision and big-diameter and foldable intraocular lens were implanted. Safety in the operations and postoperative therapeutic effects were observed. No posterior capsular rent, hyphema and fulminant choroid epistege hemorrhage were observed. Postoperative uncorrected or corrected visual acuity better than 0.5 was 54.41% after one day, 70.59% after one week and 82.35% after one month. Better than 1.0 was 8.82% after one day, 25% after one week and 41.18% after one month. Treating cataract with high myopia by small incision phacoemulsification combined with big-diameter and foldable intraocular lens implantation have the advantages of faster recovery and fewer complications. But it requires better surgery skills. The improvement of postoperative visual acuity is related to fundus changes inducedby high myopia.
