特重型颅脑损伤的救治总结

目的：总结特重型颅脑损伤的救治经验。方法：回顾性分析本科5年来收治的特重型颅脑损患68例。结果：死亡41例（60％），经手术治疗的50例，存活27例（40％），恢复良好8例，中残9例，重残10例。未经手术治疗的18例患无一生存。结论：加强对以颅底损伤为主的重型颅脑损伤患伤情变化的观察，积极行手术减压及生命支持，维持有效的脑血流灌注和脑供氧，改善脑微循环，预防和控制并发症，是提高特重型脑损伤患救治成功率和生存质量的重要手段。

Summary of management for the patients with extra severe traumatic brain injury

Objective To summarize the therapeutic experience of 68 patients suffering from extra severe traumatic brain injury (TBI). Methods The data of 68 cases with the extra severe TBI was analyzed retrospectively .Results There were 41(60%) patients dead in all of 68 cases. Fifty cases were operated and 27 (40%) survived in which 8 had good recovery, 9 had moderate disability and 10 had severe deficits. There was no survival in non operative group of 18 cases. Conclusion In order to improve the ratio of survial and the quality of living in patients with extra severe TBI, some important measures should be taken in the treatment including having actively operational decompression and life sustentation, maintaining the brain perfusion and oxygen supply effectively, improving the cerebral microcirculation as well as preventing and controlling complication .
