阻塞性睡眠呼吸暂停低通气综合征患者血清维生素D水平的变化及其临床意义

摘    要:目的观察阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清维生素D的表达水平,探讨维生素D在OSAHS引起2型糖尿病中所起的作用。方法选取单纯OSAHS患者27例,轻度组9例,中度组8例,重度组10例;OSAHS合并2型糖尿病患者14例;单纯2型糖尿病患者14例;单纯肥胖者墙例。均行整夜多导睡眠监测(PSG)及空腹血糖(FPG)、糖化血红蛋白(HbAlc)测定,ELISA法检测外周血血清维生素D水平,了解血清维生素D在OSAHS患者糖代谢紊乱中的作用。结果①与肥胖对照组相比,糖尿病组、OSAHS组、合并组血清维生素D水平明显降低(P〈0.05);合并组血清维生素D明显低于糖尿病组(P〈0.05);②维生素D与BMI、AHI、HbAlc、FPG呈负相关(r=-0.309,-0.516,-0.421,-0.458,均P〈0.05),与最低血氧饱和度(SaO2min)呈正相关(r:0.349,P〈0.05)。结论OSAHS可引起血清维生素D水平降低,其可能在OSAHS致糖代谢异常中起重要作用。

Abstract:Objective To observe the changes of serum vitamin D levels in patients with obstructive sleep apnea-hypopnea syndrome ( OSAHS ) , and then explore the role of vitamin D in the pathogenesis of OSAHS with diabetes. Methods To select 27 patients with OSAHS, these patients were divided into mild group (9 cases), moderate group(8 cases)and severe group (10 cases). To select 14 OSAHS patients with type 2 diabetes, 14 patients with type 2 diabetes and 19 simple obesity patients. All patients received poly sommo graphy(PSG) all night and HbA1C, fasting plasma glucose(FPG) levels were measured. Serum vitamin D levels were tested by enzyme-linked immunosorbent assay(ELISA). To understand the changes of vitamin D levels with OSAHS. Results (①)Compared with the control group, the levels of vitamin D in the simple type 2 diabetes group, the OSAHS group and the OSAHS patients with type 2 diabetes group obviously decreased. The vitamin D level in the OSAHS patients with type 2 diabetes group was significantly lower than that in the simple type 2 diabetes group. (2)Vitamin D was negatively related to BMI, AHI, HbA1C, FPG(r = -0. 309, -0.516, - 0. 421, - 0. 458, P 〈 0.05) , and positively related to LSaO2 (r = - 0. 349, P 〈 0.05). Conclusion OSAHS induces the decreased expression of vitamin D. Vitamin D may play an important role in the development of glucose metabolism disorder caused by OSAHS.
