直接冠状动脉内支架术的初步应用

目的　比较两地不同时期支架术应用的差异 ,同时比较直接支架术的即刻和长期效果。方法　选择 1998年 3月至 1999年 2月南京医科大学附属第一医院 2 0 0例和 2 0 0 0年 6月至 2 0 0 2年 3月山西医科大学第一医院 170例冠状动脉介入治疗病例 ,分析冠状动脉内支架植入术的应用率和直接支架术的使用率 ,观察手术的即刻和长期效果。结果　近年来冠状动脉内支架植入有增加趋势 ( 71%对 91% ) ,随着技术的熟练和球囊预装支架设计的改进 ,应用直接支架术明显增加 ( 5 %对 2 0 % )。结论　冠状动脉内支架植入术可降低手术早期的危险性 ,选择得当的病例施行直接支架术具有较高的即刻成功率和较理想的长期效果

Clinical application of direct coronary stent implantation

Objective To compare the differences of coronary stent implantation in two cities during different periods and assess the results of direct stent implantation for short and long terms.Methods Two hundred cases who had percutaneous coronary intervention (PCI) procedure in the first affiliated hospital,Nanjing Medical University during March 1998 to February 1999,and 170 cases who had it in the first hospital,Shanxi Medical University during June 2000 to March 2002 were chosen subjects.Analysis of application rates of standard coronary stenting and direct stenting,their treatment outcome were observed during in hospital and follow up.Results The use of coronary stent implantation is getting an increase.Along with the procedure more skilled and the design of prefabricated balloon stent more improved,the direct stent implantation had an increase in quantity.Conclusion Coronary stenting has the merit in reducing the risk of early operation,a chosen case receiving the direct stent implantation may get a higher instant success rate and an ideal long term result.
