膀胱癌抗原、核基质蛋白对膀胱癌的诊断价值

目的　评价膀胱癌抗原 (BTA)、核基质蛋白 (NMP2 2 )在诊断膀胱癌中的应用价值。方法　应用酶联免疫吸附试验 (ELISA)检测泌尿道疾病患者和膀胱癌患者尿中BTA、NMP2 2的浓度 ,并与正常组进行对比 ,利用ROC曲线 ,评价BTA、NMP2 2在诊断时的价值。结果　膀胱癌患者的BTA、NMP2 2含量与非膀胱癌组及正常组差异均有显著性 (P <0 .0 0 1) ,当BTA的临床判断值为 12 .4ng/ml时 ,诊断灵敏度为 75 .0 % ,特异性为83.2 % ;当NMP2 2的临床判断值为 11.2U/L时 ,诊断灵敏度为 92 .9% ,特异性为 93.3%。两指标的联合应用可提高检测的灵敏度。NMP2 2对膀胱癌的诊断价值优于BTA ,BTA与膀胱癌的病理分期相关。结论　尿中BTA、NMP2 2检测可用于膀胱癌的诊断

The value of the determination of bladder tumor antigen and nuclear matrix protein 22 in the diagnosis of bladder cancer

Objective To evaluate the value of the concentration in urine of bladder tumor antigen(BTA) and nuclear matrix protein 22(NMP22) in the diagnosis of bladder cancer(BC). Methods To detect the concentration of BTA and NMP22 in the urine from the patients with urogenital diseases and bladder cancer by ELISA, and compare with that from the healthy persons. The receiver operating characteristic curve was used to evaluate the predict value of the two tests. Results The concentrations of BTA and NMP22 had significant difference between BC and non BC groups( P <0.001), when the cutoff value of the BTA was 12.4 ng/ml, the sensitivity was 75.0%, the specificity was 83.2%. when the cutoff value of the NMP22 was 11.2 U/L, the sensitivity was 92.9%, the specificity was 93.3% . Combination test of BTA and NMP22 could increase the sensitivity. NMP22 test was more valuable than BTA test for the diagnosis of BC. Conclusions Determination of the concentration of BTA and NMP22 in the urine could help to the differential diagnosis of BC.
