定量组织速度成像和组织追踪成像评价肥胖儿童左心功能

摘    要:目的:应用定量组织速度成像和组织追踪成像评价轻~中度单纯肥胖儿童左心功能状况。方法:30例单纯肥胖儿童(肥胖组)及27例正常体重儿童(正常组),通过M型超声获得左室射血分数(LVEF)、左室短轴缩短率(LVFS);脉冲多谱勒超声获得二尖瓣口血流快速充盈速度(E)、左房收缩期速度(A),计算E/A;定量组织速度成像和组织追踪成像测量后间隔、左室侧壁、前壁、下壁、前间隔和后壁二尖瓣环处收缩期峰值速度(Vs)和峰值位移(Ds),舒张早期峰值速度(Ve)、左房收缩期峰值速度(Va)、计算Ve/Va。并比较两组间各参数。结果:肥胖组LVEF、LVFS、E、A、E/A都在正常范围内,与正常组相比无统计学差异;各室壁二尖瓣环处的Vs和Ds两组间差别有显著性意义,6个室壁的平均Vs和Ds两组间差别也有显著性意义;除少数室壁外,各室壁的舒张速度Ve、Va、Ve/Va以及6个室壁的平均Ve、Va、Ve/Va差别亦有显著性意义。结论:单纯肥胖儿童左室整体收缩与舒张功能均减低,定量组织速度成像和组织追踪成像能够定量单纯肥胖儿童左心功能改变。

Assessment of left ventricular function in children with simple obesity by quantitative tissue velocity imaging and tissue tracking imaging
Abstract:Objective:To evaluate the left ventricular (LV) function in children with simple obesity by quantitative velocity imaging (QTVI) and tissue tracking imaging (TTI).Methods:30 children with simple obesity (obesity group) and 27 children with normal body weight (control group) were included in the study.Left ventricule ejection fraction (LVEF) and left ventricular fraction shortening (LVFS) were detected by M-mode echocardiography;transmitral flow velocity during early diastole (E) and atrial contraction (A) were measured by Doppler echocardiography and E/A ratio was calculated;the peak displacement (Ds),the peak velocity of systolic (Vs),early diastole (Ve) and atrial contraction (Va) from posterior septum,lateral,anterior septum,posterior,anterior and inferior wall were measured and Ve/Va was calculated.The parameters of two group were compared.Results:There was no significant difference in LVEF,LVFS,E,A,E/A between obesity group and control group;the values of Vs and Ds from annulus of mitral valve,septums had significant difference between two groups.Conclusion:The left ventricular systolic and diastolic function is impaired in children with obesity,which can be detected by QTVI and TTI.
