胆结石与消化性溃疡的关系

目的：探讨胆结石与消化性溃疡的关系。方法：对胆结石患者行胃镜和幽门螺杆菌（Hp）感染检查的随机对照分析。结果：胆囊结石患者的反流发生率为68．8％,对照组为40．2％,胆囊结石与反流密切相关。胆总管结石患者的十二指肠溃疡发生率为16．3％,对照组为2．3％,有统计学意义,胆总管结石可能参与十二指肠溃疡的发生。三种胆结石患者的Hp阳性检出率分别为54．3％,51．2％,40．2％,比较无统计学意义,提示胆结石患者溃疡发生和Hp感染不相关。胆结石组的消化性溃疡发生率为11．6％,对照组为3．0％,差异有统计学意义,前者十二指肠溃疡发生率明显高于后者,而胃溃疡发生率和对照组比较差异无统计学意义。结论：胆囊结石与反流密切相关,胆总管结石患者多合并十二指肠溃疡,肝胆管结石、胆总管结石、胆囊结石等三种胆结石中的消化性溃疡与Hp无关。

The relation between cholelithiasis and peptic ulcer

Objective： To investigate the relation between cholelithiasis and peptic ulcer. Methods： The patients with cholelithiasis were contrast analyzed randomly,who were checked by gastroscope and helicobacter pylori（Hp）. Result ：The incidence rate of backflow in patients with cholelithiasis was 68.8% ,which was 40.2% in control group, so cholelithiasis had close correlation with backflow. The incidence rate of duodenal ulcer in patients with choledocholith was 16.3% ,which was 2.3% in control group,with significant statistics difference,choledocholith may contribute to the incidence of duodenal ulcer. The positive detection rate of Hp in patients with the 3 type choleflithiasis was 54.3 %, 51.2 % and 40.2% separately,without statistic difference ,which proved that the incidence of ulcer in patients with cholelithiasis did not correlate with Hp infection. The incidence rate of peptic ulcer in cholelithiasis arm was 11.6 % ,which is 3.0 % in control arm,with significant difference,the incidence rate of duodenal, ulcer ,not gastric ulcer in the former was obviously higher than that in the latter, whereas the gastric ulcer. Conclusion：Cholecystolithiasis has close correlation with backflow,and the patients with choledocholith often suffers from duodenal ulcer, the incidence of ulcer in patients with hepatolithus,choledocholith and cholecystolithiasis do not correlate with Hp infection.
