重复肾的超声诊断价值

目的：探讨二维超声及彩色多普勒显像(CDI)检查对重复肾的诊断运用价值。方法：对16例经手术及临床证实的重复肾患者的超声表现进行回顾性分析。结果：重复肾超声声像图表现可分为三类：1．肾窦被一形似柱状低回声结构分为上下相互独立的两部分，其余形态与正常肾脏无明显差异；2．上或(和)下位肾因引流不畅导致相应肾脏积水；3．上位肾重度积水，呈一个球形无回声区。运用CDI能够观察到有功能的上下位肾脏向膀胱腔内喷尿的彩流信号。结论：超声对重复肾具有很好的诊断价值。彩色多普勒显像观察膀胱腔内喷尿的彩流信号有利于重复肾的诊断，并有助于寻找异位的输尿管开口。当上位肾功能减退或丧失时，超声显像优于X-线静脉肾盂造影。

Value of Ultrasound for Diagnosis of Double Kidney

Objective: To assess the value of two-dimensional ultrasound and color Doppler imaging (CDI) in diagnosis of double kidney. Methods- The ultrasonic features of 13 cases with double kidney which was confirmed by operation and clinic were analysed retrospectively. Results: The ultrasonic characteristics of double kidney could be devided into three classes: (1) Renal sinus was separated into two parts by a low echostructure column and others were normal. (2) The lower position kidney or upper position kidney revealed hydronephrosis because of obstructed drainage. (3) The upper position kidney had more severe degree of hydronephrosis, as a no echo globular area. CDI could demonstrate color flow mictions of lower and upper position kidneys indicating their functions. Conclusions: Two-dimensional ultrasound possesses excellent value for the diagnosis of double kidney. CDI has advantages in observing the mictions in the orifice of ureters in the bladder, which proves the function of the double kidney. Ultrasound is prior to excretory urography in the diagnosis of double kidney in the situation that the upper position kidney has lost function or is hypofunctional.
