曲普瑞林和孕三烯酮预防子宫内膜异位症术后复发的效果比较

目的：对比研究曲普瑞林和孕三烯酮预防子宫内膜异位症术后复发的效果。方法：选取行腹腔镜保守手术的子宫内膜异位症患者80例，随机分为观察组与对照组。两组患者均行腹腔镜保守手术治疗。观察组患者术后月经来潮后第1 d予以曲普瑞林3.75 mg肌注，每4周1次，疗程6个月。对照组患者术后月经来潮后第1 d予以口服孕三烯酮2.5 mg，每周2次，连用6个月。观察并比较两组患者治疗后的治疗疗效、2年内的复发率和妊娠情况。结果：观察组患者的临床总有效率明显高于对照组（95.0％，80.0％）（χ^2＝4.11，P＜0.05）。治疗后随访2年，观察组患者复发4例（10.0％），对照组患者复发11例（27.5％），观察组患者的复发率明显低于对照组（χ^2＝4.02，P＜0.05）。观察组21例不孕患者治疗后2年内14例成功妊娠，对照组19例不孕患者仅7例妊娠，观察组治疗后2年内妊娠率明显高于对照组（P＜0.05）。结论：曲普瑞林治疗子宫内膜异位症术后的疗效确切，能明显降低其复发率，提高妊娠率，是一种预防子宫内膜异位症术后复发安全有效的药物。

Curative effect of triptorelin and gestrinone on the prevention of postoperative reoccurrence of endometrio-sis

Objectives：To discuss the curative effect of Triptorelin and Gestrinone on the prevention of postoperative reoccurrence of endometriosis （EMT）.Methods：80 patients with EMT,who were treated with Lapa-roscopic conservative operation,were selected and divided into observation group and control group at random.The patients in both groups were given laparoscopic conservative operation.The patients in observation group were given 3.75mg Triptorelin by intramuscular injection upon the first day of menses after the operation for every 4 weeks for a course of treatment of 6 months.The patients in control group were given 2.5mg Gestrinone through the mouth upon the first day of menses after the operation,twice a week for a course of treatment of 6 months.The curative effect, reoccurrence rate within 2 years and pregnancy condition of patients in the two groups after medical treatment were observed and compared.Results：The total clinical efficiency of patients in observation group was much higher than that in control group （95.0% vs.80.0%）（χ^2 =4.11,P〈0.05）.According to 2 years’following-up after the medical treatment,4 and 11 cases of reoccurrence appeared in observation group （10.0%）and control group （27. 5%）respectively,and the reoccurrence rate of patients in observation group was much lower than that in control group （χ^2 =4.02,P〈0.05）.14 out of 21 infertile patients in observation group successfully got pregnant within 2 years after the medical treatment,while 7 out of 19 infertile patients in control group successfully got pregnant.The pregnancy rate of patients in observation group within 2 years after the medical treatment was much higher than that in control group （P〈0.05 ）.Conclusion：Triptorelin has reliable curative effect on EMT after the operation,which can obviously reduce the reoccurrence rate,raise the pregnancy rate and is a safe and effective drug to prevent the reoccurrence of EMT after the operation.
