无创双水平正压通气治疗急性左心衰竭的临床疗效观察

目的探讨无创双水平正压通气（BiPAP）治疗急性左心衰竭的临床效果.方法收集80例急性左心衰竭患者,随机分为观察组和对照组各40例,均给予常规治疗,观察组经面罩行BiPAP,对照组采用鼻导管吸氧,比较二者临床疗效.结果治疗后6h,两组患者PaO2、PaCO2和SaO2较治疗前均改善（P＜0.05）,但是观察组改善幅度更明显（P＜0.05）.观察组患者总有效率95.00％,明显高于对照组的80.00％,差异有统计学意义（P＜0.05）.观察组患者BiPAP通气12～24h,平均（8.29±2.07）h后停止呼吸机脱机时间,无呼吸机相关性肺炎发生.结论BiPAP辅助治疗急性左心衰竭可迅速改善患者临床症状,疗效确切,安全可靠,值得临床推广应用.

The Clinical Effect Observation of Noninvasive Double Positive Pressure Ventilation Treatment for Acute Left Heart Failure

Objective To explore the clinical effect of BiPAP for acute left heart failure. Methods Collect 80 patients with acute left heart failure, each 40 cases were randomly divided into observation group and control group, both group were given routine therapy, the observation group were treated by BiPAP through the mask line, the control group treated by oxygen inhalation through nasal catheter, compare the two clinical curative effect. Results After treatment for 6h, PaO2, SaO2 and PaCO2 of both the two groups were improved compared before treatment （P 〈 0.05）, but the observation group has improved obviously greater （P 〈 0.05）. The total effective rate of the observation group was 95.00%, significantly higher than the control group 80.00%, the difference present statistically significant （P 〈 0.05）. Patients of the observation group treated by BiPAP for 12 - 24h, the average off-line time is （8.29 ＋ 2.07） hours, ventilator associated pneumonia was not occurred. Conclusion BiPAP adjuvant therapy treat can rapidly improve the clinical symptoms for acute left heart failure, which is safe and reliable with curative effect, worthed to be clinical popularized and applied.
