双侧平衡去骨瓣减压治疗重型闭合性颅脑损伤近期疗效观察

目的探讨双侧平衡去骨瓣减压治疗重型闭合性颅脑损伤的临床效果及安全性.方法选取符合标准的患者60例,随机分为观察组和对照组各30例,观察组采用双侧平衡去骨瓣减压术,对照组采用传统的单侧标准外伤大骨瓣减压术,比较二者临床疗效.结果治疗前两组患者颅内压比较差异无统计学意义（P＞0.05）;术后1d、3d颅内压较治疗前均降低,但是观察组下降幅度更大,差异有统计学意义（P＜0.05）.观察组患者术后并发症发生率明显低于对照组,差异有统计学意义（P＜0.05）.观察组患者生存质量明显优于对照组,差异有统计学意义（P＜0.05）.结论双侧平衡去骨瓣减压治疗重型闭合性颅脑损伤最大程度降低颅内压,临床疗效可靠,术后生存质量高,值得临床推广应用.

Short-term Curative Observation of Bilateral Equilibration Decompressive Craniectomy in the Treatment of Sever and Closed Head Injury

Objective To probe into the clinical efficacy craniectomy in the treatment of sever and closed head injury. observation group （30 eases） and control group （30 eases） and safety of bilateral equilibration decompressive Methods 60 patients were divided randomly into The observation group was treated with bilateral equilibration decompressive craniectomy, the control group was treated with unilateral standard trauma decompression of greater bone flap, then the clinical efficacy was compared. Results Before the treatment, intracranial pressure in the two group had no significant difference（P〉0.05）, intracranial pressure of the observation group dropped dramatically on the first, third day after the surgery（P〈0.05）. Post-operative complication rates in the observation group was obviously lower than that in the control group, quality of life in the observation was obviously better than that in the control group, and the difference was statistically significant（P〈0.05）. Conclusion Bilateral equilibration decompressive craniectomy in the treatment of sever and closed head injury is worthy of clinical application, for the advantages of maximum reduction of intracranial pressure, reliable clinical efficacy, high quality of lives after surgery.
