逆行交锁髓内钉对股骨髁上及髁间骨折的治疗

摘    要:目的:总结临床应用逆行交锁髓内钉治疗股骨髁上及髁间骨折的经验。方法:回顾性分析2001年1月~2007年6月本院收治的股骨髁上及髁间骨折30例患者的临床资料,采用切开复位,逆行交锁髓内钉内固定治疗。采用美国创伤骨科学会(OTA)的标准骨折分类AO分类法:A1型14例,A2型5例,A3型5例,C1型3例,C2型2例,C3型1例。结果:随访平均14±4.4月(6月~2年),骨折平均愈合时间20±1.8周(17~22周)。根据Neer等膝关节功能评分标准:优26例,良3例,可1例。结论:逆行交锁髓内钉治疗股骨髁上及髁间骨折疗效好,大部分患者可通过早期持续被动性活动机锻炼,使膝关节功能达到满意的效果。

Abstract:Objective :To summarize the clinical experience about the treatment of epicondyle and intercondyle femoral facture with the application of retrograde interlocking bone nail. Method: A retrospective study of 30 cases with epicondyle and intereondyle femoral faeture from Jan, 2001 to June, 2006, all of which are applied open reduction and internal fixation with retrograde interlocking bone nail. (They are classified according to the OA categorization: type Al,14 cases; type A2,5 cases; type A3,5 cases; type Ci,3 cases; type C2,2 cases; type C3 1 case). Results: Followed up with 6 moths to 2 years(14±4. 4 moths), the average union time of these facture patients is about 17 to 22 weeks(20±1.8 weeks). The effects evaluated by Neer etc. knee joint funation scoring standard is: excellent,26 cases; fine, 3 cases ; common, 1 case. Conclusion: Retrograde interlocked bone nail is a good approach to treat epicondyle and intereondyle femoral faeture,most of the patients can get satisfactory effects via early CPM knee joint exercise.
