根管治疗后不同时机牙周治疗对牙周牙髓联合病变患者牙周指标及龈沟液炎性因子的影响

探讨根管治疗后不同时机牙周治疗在牙周牙髓联合病变患者中的应用价值。方法 选取 我院2022年1月-12月收治的90例牙周牙髓联合病变患者，按随机数字表法分为观察组与对照组，每组45 例。对照组在进行根管治疗后3周予以牙周治疗，观察组在根管治疗后1周予以牙周治疗，比较两组临床 疗效、牙周袋探诊深度（PD）、菌斑指数（PLI）、白细胞介素-8（IL-8）、白细胞介素-6（IL-6）、 疼痛率视觉模拟评分法（VAS）评分]及并发症发生率。结果 观察组治疗总有效率为97.78%，高 于对照组的80.00%（P ＜0.05）；观察组PD、PLI低于对照组（P ＜0.05）；观察组IL-8、IL-6水平低 于对照组（P ＜0.05）；观察组疼痛率为8.89%，低于对照组的37.78%（P ＜0.05）；观察组并发症发生率为 4.44%，低于对照组的20.00%（P ＜0.05）。结论 牙周牙髓联合病变患者在根管治疗后1周予以牙周治疗能 提高治疗效果，改善牙周指数、龈沟液炎性因子，减轻疼痛感，降低并发症发生几率，值得临床应用。

Effects of Periodontal Therapy at Different Times After Root Canal Therapy on Periodontal Indexes and Inflammatory Factors in Gingival Crevicular Fluid of Patients with Combined Periodontal-endodontic Lesions

To explore the application value of periodontal therapy at different times after root canal therapy in patients with combined periodontal-endodontic lesions. Methods A total of 90 patients with combined periodontal-endodontic lesions admitted to our hospital from January to December 2022 were selected and divided into observation group and control group according to the random number table method, with 45 patients in each group. The control group was given periodontal treatment at 3 weeks after root canal treatment, and the observation group was given periodontal treatment at 1 week after root canal treatment. The clinical efficacy, periodontal pocket probing depth (PD), plaque index (PLI), interleukin-8 (IL-8), interleukin-6 (IL-6), pain rate visual analogue scale (VAS) score] and complication rate were compared between the two groups. Results The total effective rate of treatment in the observation group was 97.78%, which was higher than 80.00% in the control group (P <0.05). PD and PLI in the observation group were lower than those in the control group (P <0.05). The levels of IL-8 and IL-6 in the observation group were lower than those in the control group (P <0.05). The pain rate of the observation group was 8.89%, which was lower than 37.78% of the control group (P <0.05). The incidence of complications in the observation group was 4.44%, which was lower than 20.00% in the control group (P <0.05). Conclusion Periodontal treatment at 1 week after root canal therapy can improve the therapeutic effect, periodontal index, gingival crevicular fluid inflammatory factors, and reduce the pain and the incidence of complications in patients with combined periodontal-endodontic lesions, which is worthy of clinical application.
