TACE联合氩氦冷冻消融治疗原发性肝癌效果观察

目的探讨肝动脉化疗栓塞术(TACE)联合氩氦冷冻治疗原发性肝癌的效果及患者细胞免疫功能变化。方法选择42例原发性肝癌患者随机分为2组:A组共22例,行TACE联合氩氦冷冻消融术;B组共20例,单纯行TACE治疗。观察2组治疗有效率及血清AFP水平变化,同时分别检测患者治疗前、治疗后2周T细胞亚群CD3+、CD4+、CD8+的变化;同期选择正常健康体检者30名为对照组。结果 TACE联合氩氦冷冻治疗的有效率明显高于单纯TACE治疗组,TACE联合氩氦冷冻治疗患者AFP水平显著低于单纯行TACE组患者;原发性肝癌患者治疗前CD3+、CD4+、CD4+/CD8+T细胞值均低于正常对照组(P均<0.05),CD8+T细胞值高于正常对照组(P<0.05);TACE联合氩氦冷冻治疗后患者CD3+、CD4+、CD4+/CD8+T细胞值显著增加(P均<0.05),CD8+T细胞显著下降(P均<0.05);单纯TACE治疗后患者CD3+、CD4+、CD4+/CD8+T及CD8+T细胞值变化均不显著(P均<0.05)。结论 TACE联合氩氦冷冻治疗原发性肝癌的疗效优于单纯TACE治疗,TACE联合氩氦冷冻治疗能够提高原发性肝癌患者的细胞免疫功能。

The efficacy of primary hepatic carcinoma treated by TACE combined with argon-helium cryoablation

Objective To investigate the efficacy and variations of cellular immunity function in patients with primary hepatic carcinoma treated by TACE combined with argon-helium cryoablation.Methods The 42 primary hepatic carcinoma patients were randomly divided into two groups: group A(n =22),treated by TACE combined with argon-helium cryoablation and group B(n=20),treated with alone TACE.The efficacy and the level of AFP in serum of patients were observed.The level of T-cell subsets of patients,CD3,CD4 and CD8,was detected by flow cytometry before treatment and 2 weeks after treatment,respectively.The immunity function of 30 healthy people were evaluated as control.Results The respond rate of group A was higher than group B,and the level of AFP of patients decreased in group A than that of in group B notably.CD3+,CD4+ and CD4+/CD8+ T cell in primary hepatic carcinoma patients were lower than control group(P<0.05) before treatment,and CD8+ T cell were higher.After treatment,CD3+,CD4+ and CD4+/CD8+ T cell were increased(P<0.05),and CD8+ T cell decreased significantly(P<0.05) in patients of group A.There were no changes in group B(P>0.05).Conclusion The treatment of TACE combined with argon-helium cryoablation has more efficacy than TACE alone.TACE combined cryoablation treatment enhances the cellular immunity function of primary hepatic carcinoma patients.
