运用药学服务单对高血压患者进行患者教育的案例讨论

目的:探讨药学服务单的设计与患者教育的方式。方法:通过对2例存在不同问题的老年高血压患者应用药学服务单进行患者教育,探讨设计药学服务单及其在患者教育的实际应用中遇到的问题和解决方法。结果:药学服务单在提高患者用药依从性、建立药患关系、在治疗团队中找到临床药师的位置起到非常好的作用。结论:利用药学服务单进行患者教育是临床药师开展药学服务的一个很好的工作模式,值得推广。

Exploration of the Application of Pharmaceutical Care List in Patient Education for Hypertensive Patients

OBJECTIVE： To explore the way of patient education and the design of the pharmaceutical care list. METHODS： 2 elderly patients with hypertension were provided patient education with pharmaceutical care list, and pharmaceutical care list was designed and the problems about the application of it in patient education and solutions were discussed. RESULTS： The pharmaceutical care list played good role in improving medication compliance, establishing a doctor-patient relationship and finding our own place in treatment groups. CONCLUSIONS： The pharmaceutical care list is an effective mode of pharmaceutical care provided by clinical pharmacists for patient education, and it is worthy of promoting.
